3. Problem Details
Stuttering begins at the title screen and doesn't ever stop, through menus, gameplay etc. Only small "jumps" and the framerate (according to fraps) is at least 60fps at all times.
Happens ALWAYS but is more prevalent when the resolution is LOW (is always annoying though).

Here's what happens when I press Alt-Tab whilst in windowed mode (my most successful "fix" though I can't then control the game):
Graphics are set to minimum settings and 640x480 resolution (minimum).
System is able to run games such as L4D2 at 1920x1200 resolution with max settings and a solid framerate.
List of things I have tried to fix this problem (which makes the game pretty much unplayable):
- Switch to windowed mode with minimum resolution
- Disable sound hardware acceleration
- Reduce video hardware acceleration as far as it would go (and ME would still run)
- Upgrading video card drivers
- Changing CPU priority
- Changing CPU affinity so it will only run on 1 core
- Disabling sound entirely
- Disabling PhysX
- Disabling SLI
- Deleting, redownloading, and reinstalling Mass Effect
- Defragmenting the HDD
- Disabling smoothed framerate in config file
I've probably tried more things that I can't think of now.
Please help.

If you have already updated your video card drivers, you know it's not RAM related, Heat related or HDD related (page file, not enough swap space, you've defragged etc etc)
Just a shot in the dark. If the screen goes black and there is stuttering it could be codec related. If you have installed "codec packs" or "media packs" try uninstalling them OR looking for updates. (K-Lite/CCCP (FFDShow)
Check your soundcard drivers as well. Most people tend to ignore those. If you have built in sound, go to your motherboard/computer (Dell, HP etc) manufacturers website and look for an update OR conversely, uninstall it, reboot and see what windows comes up with instead OR uninstall it, reboot and reinstall from the website or CD (even if there isn't an update).
Check and make sure you don't have AntiViri running ( or SpyBot S&D etc). If you do, disable it (them).
If you try any of this, please don't forget to share your problem and how you fixed it.

I have disabled the onboard audio (Realtek High Definition Audio & Bluetooth Hands-free Audio & Bluetooth Stereo Audio) via Device Manager.
I have disabled AVG from MSConfig so it's not running AT ALL.
Screen doesn't go black - look at the video. The stuttering starts when the engine does and it displays the 3D globe. I will see if I can uninstall codecs but I doubt if that will be the problem.
I've played it on MAX settings (1920x1200 res) and FRAPS tells me I get a nice framerate of just over 30fps. But it still stutters just the same. I can't make the stuttering any better or worse OTHER THAN by giving some other program the focus whilst it's in windowed mode.

Just rebooted the system in diagnostic mode via MSConfig - same problem occurs.
