Yes it is real, I have it on one of my games, its found at the end of Tali's mission but you need to play her mission on Hardcore or Insanity, just before you land on her planet make sure your difficulty is on Hardcore+ and it will be found near the entrance of the room Tali is in.

I have one too!

I completed it on insanity and got one!!

Its on Talis mission i believe when you are on the bridge fighting the Geth and to the right of the big geth at the end.
