I had the same problem on the crashed ship mission (the ship hanging over the cliff ledge), my character was going down a ramp in one part (had to reload the mission) and going up a ramp in the same mission (climbing a ledge at the top of the ramp) I somehow managed to my character back down the the floor (not sure how).
Weird floating issue.

I too have had this issue (just logged out of the game in frustration). I am on a planet (doing exploration) it is all foggy, hard to see and poof I'm stuck in the air. I didn't want to have to find my way in the place again so I'm taking a break. This isn't the first time it has happened to me. Other exploration missions (one was on a wrecked ship, had to do that one like 10 times because of one specific location right before the end).
It is a frustrating bug and has actually never happened to me in combat (knock on wood). This so far is the only bug that I cannot live with. It really effects game play.

Strange.. I've got about 80 hours or so of play time and I haven't had any trouble until a couple of days ago. Suddenly, it's happening left and right.
Almost as if Bioware issued a stealth patch through the cerberus netowrk recently and it broke something...

Same here. Many, many times in 2 playthroughs. Weapon holstered or out, with or without squaddies following Shepard up. It's happened while taking cover and while just running past an object.

There IS no fix or workaround for that. The game needs a thorough patchwork from Bioware. Which i hope they are doing 'cause it's worth it and we the buyers deserve it.

Funny workaround on the crashed ship: run any ramp backwards! Yes, I know, it's silly, but at least I could complete the mission.

No doubt about that. Since this is on PC though, I'd suggest modding a hotkey to toggle ghost. If you get stuck, hit it and move away from the object, then disable it again.

I experience this issue as well from time-to-time. I first encountered this at the Citadel's souvenir shop, so I don't believe it's exactly related to the cover mechanic. Happened right after I bought a fish and walked towards the counter. Luckily, buying said fish created an auto-save, so I didn't lose much time at all.
