 I would like to have a save for ME3 where shepard and preferably the whole crew is dead.  as a result, I thought I'd start a thread where we list suggestions as to what choices lead to the death of the whole team, until a such save file exists or someone confirms a list of the decisions necessary.

Is there a way to unlock the IFF mission without gaining anyone's loyalty?
When i sent someone back to escort Kelly and the Dr, a loyal Tali survived, the play before that a normal Grunt died.

Having only the ships scanner and fuel upgrades:  Thane, SubZero and Legion bought the farm

Taking Tali, Miranda & Samara as my biotic specialist resulted in Tali's death, Mordin and Miranda with Samara as the biotic resulted in Mordin's death.  Both times Shep was romancing MIranda, and Samara was not loyal.

At the final split up, my team loyal team of Garrus and Miranda survived, but Mordin and Samara die.  My normal team of Samara and Grunt died, however the other group Garrus, jacob, and Miranda make it back to the ship with only Jacob's loyalty left to go.

Yes, you just have to perform any five missions, loyalty or otherwise.
