So at the end of the game when it asks if you want to continue playing or do a NG+, if you continue playing are you then unable to do a NG+ with that save file?

ANSWER:
You can do the New Game+ option and continue your game while still being able to pick up where you beat the game in ME3, or pick it up where you last saved in the NG+.
However, if you opted to not do the NG+, you have to redo the mission and win again to do the NG+.

Erm, I mean if you continue playing you can keep questing and carry it over to ME3. If you deccide not to continue, and do a New Game+, then you have to actually beat the game again to carry it over. It's a better idea to keep playing, finish the game 100%, and carry it over from the last continued game save point.
