I.E: Don't make a Loyal character the leader of anything. For example, I set it up as Me/Jack/Thane, Miranda as Squad Leader2 & Legion as my tech. I lost Legion as my Specialist because Miranda was the leader of the 2nd squad, but she was Normal loyalty. Nobody else died, and I didn't use Miranda again after that.

i had her leading both teams and it went well, nobody died and i had pretty much everyone exept jack and legion defend the door while i was off killing the damn reaper.. curiouser and curiouser...

Not for me the first time. Had all the upgrades, everyone was loyal, and Mordin died on me during one of the final cutscenes (he was lying on the ground as collectors ran past). Reloaded to right before I had to choose who to take with me, and took him - everything went fine and got the achievement for full survival.

So it seems you need everyone loyal to you to have the specialist not die at the beginning and not lose someone in the subsequent battles. Also sounds like having a non loyal member in command of the second squad results in a LOT of casualties.

I thought it would be really emotional to see one of my squad die. It never happened, which I'm grateful for. I like the squad a lot. But I know if I play the ending again, I will never really feel any loss when I lose someone. I thought it would be something worth experiancing.

I'm pretty sure I took my sweet time killing the boss though cause it was my first time seeing it and all.
If you escort your people to safety then they live to continue to the next game probably.

You get your crew back, not sure if they would survive without an escort, I assume if whoever you send to escort them is loyal they survive with no problems and the whole gang is back at the end (I sent Tali as I said). I saw it as a free saved squadmate, I figured the person I sent as an escort would have no problems and would survive the mission, thus I choose Tali. Tali, Garrus, Legion and Grunt are the party members I really don't think I could bear to lose so I took extra care to keep those ones either out of harms way or close to me.

Indeed. I took my sweet time with the endboss because Samara kept getting incapacitated, so me and Grunt had to focus all our attention on Harbringer. That, and I couldn't decide whether to shoot the eyes or the center power core.
Nobody died.

My first time I threw Garrus into the vent, and he took a rocket in the face trying to close the door. Then I put in Tali and she closed the door no problem. I checked the guide and it said a non-loyal specialist would die, but Garrus was loyal, so I guess some people just die despite loyalty.
The second part with the biotic I had to do like 5 times. The first 3 times I was putting actual soldiers like Grunt and Zaeed in charge of the second fire team, and they kept dying from a shot after getting through the doors. The 4th time I sent Miranda as the leader and used Thane for the bubble. Thane sucks. He fell at the end and Jack got carried away by the swarm, but Miranda lived. Then I did it properly, and lost nobody. Miranda got shot at the end of it, but she shrugged it off. I guess she's tougher than a krogan.
Nobody died in my final defense, and I brought Jacob and Miranda to the final boss (who looks extremely silly, by the way)
