A confusing question regarding character deaths

All characters are loyal and none have died in previous sections. Picked characters best suited for both the biotic and the tech, and hell I even picked Grunt to lead the fire team thinking that I wasn't have any luck with other characters that why shouldn't I pick the one that would, in theory, take a whole cruiser landing on him to even make him flinch...nope, died as well.

That fight is all about shields, people with shields can survive that final gunshot.

Another note, just went through the biotic bubble moment with Jack as the bubble, using Miranda and Legion...Jack died, Legion died...I mean, I can understand Legion dying, but I'm still trying to figure out the whole Jack dying bit, after-all, for such a biotic dynamo, what's so difficult about holding a bubble up against millions of mosquitoes?

Garrus, Jacob, Zaeed, Grunt, Mordin...pretty much everyone but Sammy and Miranda, as they were on my team during that time.

Jack survived the biotic bubble for me, so I suspect the mechanics of the suicide mission are more involved than simply choosing the "right" person.

Honestly I've never tried this without Samara simply because it seems like the one job she SHOULD do in the entire mission.

Deaths are based on what upgrades you got and the loyalties you have with your team-mates. As well as your own personal training of them (skill selection) and what they're best at. Your selections could kill another team mate, kill both of them, or end up killing someone later. ME2 has integrated logic, unlike other games where you can randomly select characters for assignments and they will excel depending on their level.
Also, if you take too long within the Collector base (you should just be killing and running, otherwise your tech will die, your crew will die, or other people will die due to your lack of speed). Weapon upgrades and stuff are just as vital here folks, ME2 forces you to play this game fully if you intend to have everyone survive.
For example. I chose:
Team 2 - Miranda
Tech - Legion
Biotic Bubble - Jack
Team 2 - Garrus
Escort - Jacob
Everyone but Miranda was loyal. I figured someone was going to die in the ducts regardless, so I sent in my robot. Legion died because of my selection, because of Miranda. Stuck his head into a rocket. I made a speech before running off after saving some of my crew, but I didn't choose her after that. I lost no one else, not even Chakwas and Kelly.

1. What level are they.
2. You've done everyones side-quests, and finished all conversation you can have with them? This includes declining romantic interests, or getting to good pal status. I.E: Jack tells you to stop pissing around and she doesn't want to play, and Jacob invites you to spill drinks on the citadel.
3. Normany sequences are not necessarily the only thing that will kill you via upgrades; lack of weapon prowess will slow down the other squad, and slow you down in turn. The lack of speed can kill your team mates. How upgraded are you?

I had maxed each characters specific trait and evolved it. I had all Normandy upgrades. All characters were fully loyal (loyalty missions complete, Jack returned to loyalty via Paragon switch). Of the two times I've gone through the entire sequence, Tali was my tube runner, never died. First time through, Garrus was team leader both times, sent Jacob back with crew, used Jack as biotic bubble. Garrus died. Second run through, used Grunt as team leader both times and kept Jack as biotic bubble. Grunt dies, Jack dies and takes Legion with him.

Level 27 (basically same as me, look above to see character traits maxed and evolved)

Yes to all above.

Well, considering that each character that had specific weapons had specific weapons (Grunt, Legion, etc etc) their loadouts weren't the issue. I picked the character that best suited a role (except when I put Grunt as fireteam leader), or at least best suited based upon their attributes. Based upon the character screen, all had their loyalty circle under them, all of their abilities were lit up when highlighted.

Ok, let me lay it out...

The tech is saved by the haste and intelligence of your 2nd squad leader, and whoever is actually with them is important. If you hear that they are having trouble moving along on your side, you have picked the wrong person. They need to have enough fire-power to make it through and provide adequate cover with good leadership. Jacob or Garrus are the easier choices than Miranda, who's more of a support unit. Clear the room before you open up the 8th valve, as well.
The biotic shield; if they get hit often, or too much for that matter, they will die. They will be too weak by the time you reach the entrance; I was lucky my first play through, and Jack only got hit twice. This still slowed her down considerably, and its' obvious why. I have no doubts a few more shots and she would have collapsed. I always bring a 2nd biotic with me here, as throw is extremely useful to get them to back off your biotic.

This wasn't the first portion split-off, this was after freeing the Normandy crew and starting on the biotic bubble portion. Neither time during the biotic bubble did Jack get hit, as Sammy did a lot of area throws while the Collectors were incoming. I timed the second time and it took me ~7 minutes from start to the finish cutscene, so time spent with bubble doesn't seem to be an issue.

Who did you use for valve? I used fully upgraded and loyal Legion...he died.

I was full Paragon:
This is what I did,
-My squad was always Legion and Garus.
-My tech dude climbing through the tubes was whats his face, the Salarian.
-Used Zaaed leader on boths teams (I chose him because of his military and victory over impossible odds in his bio and from talking to him.. I figured it might make a differnece)
-Miranda was my bubble escort
Garus got pwned by the seeker swarm
Im trying to figure out how to save garus, everyone else can gtfo. I want him to live, even If i die. Hes too awesome.

Rolling with Legion and Garrus is pretty much bringing two snipers into a front line assault. That is how I expected for Garrus to fall easily. Try another combo with Garrus not being in your perma-squad unless you are willing to boot Legion for someone more front-line resilience.
PS: Legion I mean.
