Is it possible to get both maxed out? Alsois there any main differance overall besides flavor? and your scars reacting accordingly?

the differences are in attitude mainly, and different results to big decisions, you get more info and get to know people better with paragon and yes the scars get worse and glowier when you're renegade.
EDIT: I seriously doubt you can max both out.

I guess with remarkable timing on my part with Sheppard's morale booster perk, I finished my first playthrough with 100% paragon and ~%60 renegade. The only time I wasn't given renegade options were between Jacob & Miranda, Samara and her daughter, and then Jack & Miranda.
