I had everyone loyal but Miranda and got No One left Behind!

Legion was my Tech

Garrus was the second team leader the whole time

Sent Jacob to escort Dr. Chakwas back to the Normandy

Samara was my Biotic Barrier

And took Samara and Thane to kill the Boss

Upgraded everything.

No one died.

Isn't it impossible to keep everyone loyal though? Jack and Miranda (and legion and Tali) both have arguments that make one of them unloyal to you.

It is not impossible, you just need a high paragon or renegade score.

As for me

Tech - Tali
Garrus for the Fire team
Escort jacob
Samara shields

Everyone was loyal

I have beaten the game 3 (normal, veteran, insanity) times with this combo and i have not lost anyone including the crew.

I went through on Normal (finished at around 26 hours, half of what I spent my first run through)

Squad Setup:

Tech Specialist = Tali
Fire Team + Distraction = Garrus
Escort = Mordin
Biotic Shield = Samara

Reaper boss: Tali + Thane

Other Notes:

- I had 100% Renegade, 40% Paragon (I imported an ME1 100% Paragon character)
- * Did all of the quests (* = I actually gave Legion to Cerberus, thus, not giving me the chance to recruit him)
- Immediately did the Omega 4 Relay after the IFF was online
- Played on Normal difficulty

End Result:
A flawless finish. Every squad member and all of the crew members (well the ones that really mattered) survived. I really thought not recruiting Legion would have affected my chances at a flawless finish, but it didn't. What it did do was screw me over from possibly making 55k credits to buy up the rest of the upgrades on Illium.

Make sure that you're loyal with EVERY teammate before you attempt the suicide mission. I also don't know if talking to your team throughout the entire game is a necessity or not. Do it anyway. People have obviously been getting different results with the perfect endings. Rather intriguing. :)

You dont need everyone to be loyal, just those in special roles  or in your party for the terminator baby. So if you dont have legions loyalty dont put him in the tunnel put tali.

tech: Legion or Tali will work perfectly
Leaders: Garrus, Miranda, or Jacob as some people have said.
Biotic: Miranda, Samara, or Jack
Escort: any loyal member
Final team: Any loyal squad member.

any person in any of those roles HAVE TO be loyal or someone will die. if the biotic is not loyal the shield will fail and you will lose a squad member you brought with you to the swarms at the end. if your final team has any unloyal members you will lose them in the fall at the end.

FWIW, here's what I did (everyone survived on my first try)

- Got all the major Normandy ship upgrades

- Did EVERYONE'S loyalty quests. It takes forever, but it's worth it.

For team specifics:

-First Half-

Mordin and Miranda /w Shepard

Legion as tech expert

Garrus led the 2nd team (and he had Thane /w him. Dunno if it's randomized or what)

-Second Half-

Jack and Grunt /w Shepard

Samara as the biotic shield

Miranda leading the second team

Thane escorted survivors

-Last Half-

Jacob and Garrus /w Shepard

I think the big major keys are the tech experts, fire squad and the biotic shield. Not only do you need the loyalty, IMHO you also need someone who's specialized in those areas. Characters that only have half-knowledge -so to speak- in some areas (Mordin for tech, Thane and Jacob for biotics) just doesn't cut it, no matter how loyal they may be. The more focused, the better. Plus with the fire squads, having characters who have had stated in-game that they've had experience (Miranda and Garrus in this case) leading groups are essential as well.

Other than that, the rest is up to you!

I had everyone loyal except Miranda (took Jack's side in their catfight) and Zaeed.

Chose Legion to go through the vents. After all, he can hack faster than any organic.

Chose Garrus as fire team leader both times because he's had experience leading teams.

Chose Mordin to escort the crew back to the Normandy because they'll need medical attention the moment they got back on-board.

Chose Samara as biotic shield because she's been practising combat biotics for hundreds of years.  Chose Thane and Grunt for the combat.

Chose Samara and Grunt in my final team because they're the most hardended combatants after Zaeed, and he wasn't loyal because I refused to sacrifice the colonists for his revenge and I didn't have Renegade or Paragon options to force him to be loyal.

Everyone survived.

I never understood why some seem to have such difficulties with this mission! On my first playthrough, I just chose what made the most sense to me, and I got everyone out alive. You just need to prepare before the final mission, and know what your squad members are and aren't capable of.

Think logically as well get them all loyal and ship fully upgraded.

when setting up team think logical ofc someone like grunt or zeead have no leadership skills to lead a squad, and the techs explain themselves.

I had all of the ships upgrades minus the med-bay, probes and fuel.

Everyone was loyal except for Jack. I took Miranda's side. I just cant resist her accent!

The first time I had Legion go in the vents

Garrus was the team leader

Thane and Miranda were my squad mates.

Fire team lead - Garrus

Escort - Grunt

Biotic spec. - Simara

Squad mates - Miranda and Thane

Final Battle - Miranda and Thane

Jack died - so I tried it again. This time I sent Mordin as the Escort for the crew and had Grunt with the diversion team. Everything else was the same. And everyone lived!!!! So it really does depend on who you pick for what.

Throughout the whole mission Mordin and Tali were my squad mates.

Legion went into the vents.

Garrus lead the diversion squad.

Jack was biotic specialist

Grunt was the escort.

Garrus was final stand group leader.

No one died though the goddamn cutscenes made it looked like a few people had a couple of times.

Infiltrator

I lost Miranda's loyalty when I decided to side with Jack during their argument; However every other squad member was loyal, also had the 3 ship upgrades needed. So this is how I was still able to get the achievement...

<<Legion- He went into the ventilation ducts.

<<Garrus- I assigned him as squad leader on both occasions.

<< Samara- I had her use the biotic barrier.

<<Zaheed- I escorted the survivors back to the ship.

<<Thane and Jack- I had them in my squad each time.

--I purposely did not assign Miranda to anything and it worked out great.

Everyone survived and the Reaper ship was destroyed!!

so far I finished game 8 times which I can't believe myself how I can play the same game that many. anyway, finally, I achieved 'no one left behind' last night (8th game with infiltrator class, female shepard). it's kind of funny because woman seems to get work done better than male shepard in my case. so I achieved all now.

now I can figure out certain pattern to survive all members. Legion is very important. you should send him as vent to solve technical problem. then second leader would be Miranda, and Garrus. Samara will solve swarm attack in last point to meet all squads for next attack. continually, you should choose die hard members like Garrus, and Samara to fnish the last reaper crap.

by the way, Jacob was failed to get all crews to get back to Normandy excepting Dr. Chiwaga. I think that Jacob is just completely crab. even Mordin can bring all crews safely.

Hardcore as Engineer

From the start:

Legion in the tunnel.
Zaeed and Kasumi with me
Garrus leading the Fireteam.
No deaths

Jack doing the Biotic bubble. (tried twice with Mirranda doing the bubble but someone always died. Probably has to be either Jack or Samara/Morinth)
Legion and Garrus with me (dual combat drones is stupidly effective)
Mirranda leading 2nd team.
Mordin on escort (he seems to always die if you dont send him back there)
No deaths

Final fight:
Zaeed and Legion with me
No deaths.

Had loyalty done on everyone. Its a good idea to get all the loyalty missions done before you go for the IFF, and once you have it you can go straight to Legion's mission. Make sure you have 5k platinum for Geth shield research cause you wont get a chance to mine after you do the Legion mission.

I'm not sure if it importent or not but i made a point of having full Paragon for this playthrough. Might not do anything but it was worth a try. Also made sure i had all proto-types researched, again, dont know if it's relevant.

Legion goes into the vents, Garrus leads first & Second squad, Samara holds up shield, Mordin escorts crew back, Tali and Legion come with me to fight the Terminator. Everyone survives.
