I sent Tali to escort the rescued crew back to the SR2 & heard that some people sent grunt for that.
I designated Jack as the Biotic barrier person, while bringing Grunt & Legion with me & set Garrus as the 2nd team leader.
For the Final I took Grunt & Miranda

I used Tali for the vents, Miranda for both times you need a second team, Samara for the protection through the bugs, and the last two for my team was Legion and the scientist, because one of the last scenes showed him die, so I took him with me. And I sent Zaeed with the survivors of my crew.

Second team - lead by Miranda all the time.
Samara - Biotic barrier
First unlock - Legion
My team throughout - Garrus & Mordin
Survivors - Grunt.
Zaeed was the only one to die, and that was because I screwed up his loyalty mission. Otherwise everyone survived.

how to keep your tech in the tube alive? i have no idea. i had all chars loyal. tali was SUPER loyal to the point of wanting to share suit time with my fem shepard. yet - took one to the faceplate.
i tried several others and they all took one ot the head if i sent them in the vent. bug?

There is a timer that you have to beat when opening up the vents. It should appear as a bar on the lower right side of the screen.

I had all the characters loyal when I started the final mission and then:
Tubes: Legion...he's basically a terminator (speaking of terminators...ahem) so I figure he can hack anything he needs to.
Second Team (1): Garrus. Just because he's a Turian gangsta.
Second Team (2): Miranda. Basically trying to think of those who've led things before.
Escort Guy: Grunt. No explanation needed.
Biotic: Samara
My team was Grunt and Jacob, then Jacob and Tali. Everyone came out without a scratch.

I finally succeeded by sending Garrus as the leader of the second team instead and using Tali in the tube.
I think that's not the trick to the first part though, I think you have to be sure of one of two things (I'm not sure which it is...)
1. You have to be absolutely sure you kill all collectors before popping open the final vent lock.
2. The "Specialist in Danger" health bar that seems to be reset after you open each vent... I think it's secretly cumulative, if the specialist dips down to a quarter of it 4 times, or half twice for example they'll die...
I'm not sure which it is...

I had (at least i thought) everyone loyal. is there any reason a character would turn neutral again by something you did? And even if they weren't loyal, would they just randomly die at the end?
I had Legion in the tunnel
Miranda and Jacob as my fire squad leaders
Samara as my biotic barrier
and Zaeed as my escort
i had nearly every weapon upgrade possible considering i played every side mission (searched EVERY planet for them) all ship upgrades (aside from med bay) started the suicide mission at level 29 (finished the game at level 30), and even exhausted every conversation Mordin had avaialbe, he said multiple times how much he's happy to be there.
no clue, anyone got any idea why it happend?

I picked Tali for the tunnels, Garrus for the 2nd team, then Jacob for the second other team, Grunt was the escort, Samara was the shield biotic and then Jack and Thane were my final team. None of my actual team died, so I got the achievement.

I had all the loyalty quests done but hadn't bothered getting Zaeed. Get Zaeed hoping doing his mission would trigger it and instead I don't get to do his loyalty before the IFF. Now, I have 2 missions to do. But that'll probably screw the crew over. But I won't get the acheivement.
I don't like Zaeed, so I think I'll just do Legion and let Zaeed die.

Second part: Garrus in second squad, Samara as biotic, Jack as crew escort.
Final: Grunt and Legion with me as always.
All survived and all were loyal, missing a few upgrades though.

Tried to use Grunt for second team, figured what the hey, the guy was genetically engineered and had his brain dumped full of random crap in order to make him the perfect military commander. Miranda ****ed, but it worked out fine.
Sent Jacob in the pipes first playthrough because it looked like another "Bioware Plot Twist of Doom" i.e. that thing where they give you a box of cute puppies and ask you which one you want to beat to death with a hammer. Jacob's okay and all, but Tali is one of the better written characters in either ME game, and Legion is Legion.
Second, with spoilers, probably send Legion in the pipes. Jacob kept whining about how uncomfortable it was and how he was dying in there any time I slowed down to take my time sniping off the Collectors, maybe a robot will be less of a wimp.

Happened to me too. Take Moradin with you on the final leg of the mission, he's not much of a "hold the line" type guy.

For the crew, it seems to be pretty simple. DON'T get the Reaper IFF signature until AFTER you have done EVERY mission you want to do before you go to the ending. Trust me, just don't do that mission until you're ready to use the Omega 4 relay in every other way. Not sure if their personal quests matter, but I know that matters.

Also, as a side note, had Mordin along with me during the final mission, as I thought there might be super-techie stuff that the super-techie geneticist needed to do (Nope, plant a bomb, go to town with a Collector laser to the eyeball.) He was the one that slid off the platform and I barely saved him. Miranda was with me and got nearly squished by a big chunk of metal, but was fine.

I did approximately the same thing.
My first playthrough, Legion as the door opener, Garrus as the leader of team 2 (twice), Samara for the biotic barrier. I sent Jacob with the survivors, and used Grunt and Jack as teammates
Worked quite well... Everyone survived, even Kelly
And i can confirm this : you don't need to kill all the collectors before you open the last vent lock... The tech guy (Legion in my case) was in danger... there were too many ennemis... so i just used tactical cloak and rushed to the last button... it worked well...
Interesting thought :
I just noticed something... At one point, when you learn about the IFF (or ITF or whatever... don't remember the exact name) you have a choice : either continue your quest on getting a nice team, or go find this IFF-thing. I think it's Jacob who tells you to go straight to the IFF, because you're gonna need it anyway, so you might aswell get it now.

Well well well... if you follow his advice, then you can't save all your crew (Kelly) AND have your whole team ready for the mission... (if you want to save your crew, you need to head to the Omega 4 relay ASAP after the collectors attacked the Normandy... And the IFF caused the attack...

Second playthrough, did Legion's loyalty, high-tailed it to the Relay. Kelly dies.
