You need to have almost everyone loyal to you, and you need to be reasonably quick opening the gates for your tech specialist. You also need to choose appropriate characters for the positions. It is possible weapon upgrades, character upgrades and character level may have a positive effect.

I had everyone loyal but Miranda, used Garus as team lead both times, Tali as tech and Samara as bubble. I researched the personal upgrades for Legion, Grunt and Mordin, and had all weapons at 3/5 to 5/5 research level.

I'm pretty sure that to get Mordin to live you need to take him with you don't leave him with the rear guard team. Salarian Geneticists are squishy. :-P

If the person you send into the tube is not loyal, they will die. Nothing you can do.

And yes, it is easy to get everyone to survive. All you have to do is do all the loyalty quests, get all the upgrades, and be sensible about your choices during the final mission.

I got almost every achievement in one playthrough.

Also, Mordin is not a tech specialist. He is the wrong choice for the mission in the tube. Use either Legion or Tali.

Grunt,Mordin and Zaeed died for me. :( I put Grunt in charge of Team2, made Zaeed take the crew back to the Normandy, and left Mordin back with the backup team.

I sent Mordin back with the crew cause he has trouble holding the line. So he was fine.

All you have to do is be smart about the choices. Miranda is the natural choice to lead any team. I sent Garrus to escort the crew, which he did perfectly. Legion went in the tube, and I used Samara for the Biotic sequence. Again, very obvious choice there.

Then how is it Legion died when I sent him into thew tubes? I had all of his upgrades and he was at max loyalty. Zaed died when he was with th other team during the whole bubble incident. What did I do wrong?

Think logically:

Who's the best tech specialists on your squad? The Quarian the AI.

Who's the powerful biotics on your squad? Subject Zero and the Justicar.

Who's good at leading teams and giving orders? Garrus and Miranda.

Who's good at defending people? While this one was harder, I sent in Grunt because he's big and invincible.

I did the same thing! He died whenever I did something else with him. He seems prone to dying. He was fine gonig back with the crew though.

for the tube part, if the person you pick to lead isn't up to the task, your tech specialist dies, I thought Zaeed's years of experience would make him an acceptable leader, but her failed me and got Legion shot in the face

He lived both when I sent him back with the crew and when I took him with me into the final fight.

Its like real life. Do things right and play smart and everyone (or at least most) will live.

This, though I sent Zaeed for the escort. Among the choices I leant most on people who had the most experience for the specific tasks. Jacob is probably pretty suitable for the task too, since he was the top LT of security at Lazarous

I thought about this too, Zaeed definately has tons of experience, but I felt like he would be reckless with lives to get the job done. I always chose Garrus to lead the teams, because he hates losing people under his command. Maybe none of this psych stuff matters, and its just as simple as loyalty and upgrades. I hope not.

Zaeed led my teams considering he is a veteran and has been doing **** like this his whole life.

I had everybody's loyalty but lost Miranda's when I sided with Jack after I blew up the Cerberus Facilty Jack stayed at. So, Legion died while we were shutting the door and Miranda died near the end. I honestly don't think there is a way for who ever goes through the tubes to survive.

There is.  There quite obviously has to be a way or else the "no one left behind" achievement wouldn't be possible.  My advice:  Move quickly.  Open the gates blocking their path as quickly as you can.

Everyone survived in my game. It's possible.

My whole squad was loyal and I did all the ship upgrades plus 95% of the other upgrades.

I used Miranda as the other squad leader, sent Tali throught the vents, had Samara create the biotic-bubble thing.  I took Miranda and Jack with me for the final battle and the rest hung back to cover us.

I picked Miranda to lead during the swarm part of the mission.  My whole squad was loyal though.
