Everytime I go into the gameplay menu it defaults without me changing the settings.  Even if I come out of the menu and go right back in it will say "Insanity".  Then within time, next set of enemies I notice them going down in a few hits, then I go look into the gameplay menu and it set to casual.  I'm guessing it's after a auto save or something.

Pretty annoying now seeing I was on a 15 hour run and wanted to get the achievement for beating the game on Insanity...  I know all I need to do now is just not go back into the menu what-so-ever but I am just wonder, has anyone else had this problem? Could it be something to do with the HDD needing a clear cache? Oh and the game is installed.
