At the scene before the credits, Shepard was looking at some sort of.. coffin? I didn't get any of my crew killed, as far as I know. What's up with that? ..That is a coffin right?

Not hers, Kelly isn't dead. I guess what I'm asking here is, is this scene in the standard ending, or is it because someone died and I missed it?

Someone died. The coffins only show when you have lost a squadmate.

Crap, I didn't get the all survived achievement. I guess someone died at the end. Problem is I don't know who. Everyone was alive up till the final squad selection before the final boss. Can someone die after that?

There can be a small scene with the collectors chasing, they'll be running by the corpse of whoever died.

It might have been legion. I found he died if you didn't use a strong enough biotic, he dies saving them.

I got the "no one left behind" this time, but it's weird. All I changed was the 2nd fire squad leader from Garrus to Jacob. Both times they survived. Final fight, I picked Miranda/Thane as opposed to Miranda/Grunt in the first time. Nothing seemed different, and yet someone died the first time and I didn't know about it.

Used Samara for the biotic. It's as strong as it gets..
