Additionally, after using advanced training a fair few times, I have lost several of my skills from the shift menu, and some of the new ones are not available. Seems to be a corruption of the save file, since no amount of reloading fixes the problem.
Really frustrating =/ I played it through a second time to get everything right, and despite doing all loyalty quests successfully both Shepherd and Grunt are broken

It wasn't always like that. I think it has to do with issues patching consoles and the demand that "If PC gets a patch, us console 'tards demand one too!".Zeuserich wrote...
SWKotOR and Dragon Age taught me that BioWare either too afraid to make patches (scared of breaking stuff that actually works) or does not have enough peoples sorting through gameplay bugs and working on fixes.

Edit: found a temporary fix at least, you can use Gibbed's save editor to make grunt loyal. My save had grunt as being not loyal meaning he would likely have died at the end. To fix it open you save, select henchman under the raw tab, and click on the box that shows up to the right of where it says collection (this opens a new window). Then select the henchman you need to fix and select powers, click on the box next to collection again (opens another window). In this window select loyalty requirement and change the current rank from zero to one, after that just save the file (back up you original one first) and load it up in game.
After you do this it will show the skill grayed out from you squad info page, but once you actually go somewhere with grunt it will fix this.

It seems if Grunt does his usual Kamikaze charge and ends up on the ground as the Thresher Maw dies, this happens. You may not have noticed during the battle because you get a bit busy and Grunt revives again straight after it dies. The only way to avoid it is to be quick to hit the medigel if he goes down and revive him before the Maw dies.
Bug? Maybe and maybe not. He's loyal because you've kept your side of the deal but he doesn't get his extra power because technically, he didn't help defeat the Maw. It's probably a bug but if not, I'd say this is the reason.

If you didn't use a Charm or Intimidate option in that conversation, you will lose the loyalty of one or the other (according to which one you side with). You must have sided with Jack. I think it's possible to regain lost loyalty, but the romance subplot with Miranda (or Jack) is ruined for good.
