Ok so my bug is even stranger but I think it's related to this one. I'm not sure how exactly it happened, but I know it happened while trying to do everything at once on Tuchanka. Unfortunately I wasn't keeping close track of what order I did things in though. After doing both Mordin and Grunt's loyalty quests I've unlocked both their costumes and abilities, however I didn't receive either of the achievements and I can't train Shepard to use their abilities. There was also no disagreement convo between them. (I'm only assuming there was one, because it just makes sense for the story.) I also killed the Thresher Maw during grunt's mission and didn't receive that achievement either. And unrelated, I also earned the Master at Arms achievement but didn't receive it, says 5/5 in the trophy case but the medal is still faded.
