Essentially, if you want them all to survive make sure they are loyal to you before going through the Omega 4 relay. When you get where you are going male sure to assign them the tasks best suited to their abilities. Otherwise screw around and let them die.

Play the game some more. He has tech powers (incinerate & neural shock)

Heh heh, the suicide mission IS good for culling out undesirables from your playthrough. And to keep the riff raff out from ME3. Although even the possibility of everyone dying WILL be enough to limit the survivors' future roles.

That and I'm hearing people claim that they'll lose somebody, reload the save and swap people's tasks around and suddenly everyone survived. Or swapping people will save others that died in other saves. I just did a few tests with reloading mine and for me its surefire that if Jack and Samara do the biotics, Tali or Legion or Mordin into the vents, and Jacob, Thane or Garrus as the escorter of your crew then everyone lives. But thats just with my game of course.
Point is, seems there's still that smidgen of luck that comes into play. I'd really, really like to know if there's some sort of random number generator in the background, or if there's a 100% surefire way of having everyone live.

Zaeed - loyal, died defending the second wave. Don't really see how, as he is supposed to be a surviving freak soldier.
Jack - loyal, survived the biotic barrier thing.
Miranda - unloyal (jealous woman!) didn't make it back to the ship...
Didn't send anyone with Chakwas, thought I did a thorough clean but I was wrong it seems...
Had Samara and Garrus with me all the time, both on loyal

You can pick Jacob to go in the vents. He even "volunteers" to go, then it brings up a screen where - I'm pretty sure - everyone is available to be chosen.

Hey I did this too! Except I had Miranda and Garrus in my first party. Then I had Legion and Thane for the biotic barrier and Legion and Garrus for the final battle...... O when Miranda got shot were you like," No!" and then really relieved when she said she was ready to go? Also, I thought Garrus and Legion died.... I was pretty sad, but then Shep turns Garrus over and he was alive and he pulls the rock off of Legion and he was alive.
O a side note was anyone else in awe by Shep's super jump..... That's one white guy without white man's disease!

I sent Mordin in the vents but he was shot closing the door and was killed
I had Jack set up the barrier and she live
Jacob was killed by a swam of the bugs
I had Garrus lead the first team no problems
Miranda lead the next team no problems
And Zaeed lead the crew back to the ship no problems
So I only had Mordin and Jacob die...both were loyal but either due to tech not fully upgraded or wrong choice for the mission they both didn't make it.
I am playing though again though so I will see how that turns out

First time:
Did Zaeed's loyalty mission immediately after my crew was kidnapped. Then scanned a few planets, talked to my crew, and took the Omega 4 relay. (I had locked about 36 hours of time played when I took the relay.)
Vent: Thane (Died)
Alternate Squad Leader: Miranda (Lived)
My squad: Samara and Garrus (Both Lived)
Crew: Half survived
Crew Escorter: Grunt (Lived)
Biotic Protection: Jack (Lived)
Alternate Squad Leader: Zaeed (Died)
My squad: Miranda and Garrus (Both Lived)
Final Squad: Miranda and Garrus (Both Lived)
Remainder Squad Leader: Jacob (I think) (Lived)
Mordin was randomly killed
3 dead total - Thane, Zaeed, Mordin
Second time: (Reload of save immediately after crew kidnapped)
Took the Omega 4 relay immediately after crew was kidnapped. It was about 34 hours of time played when I left.
Vent: Legion (Lived)
Alternate Squad Leader: Miranda (Lived)
My squad: Samara and Grunt (Both Lived)
Crew: All survived
Crew Escorter: Jacob (Lived)
Biotic Protection: Jack (Lived)
Alternate Squad: Miranda (Shot - Panic on my part for a brief second - said she was ok. Lived)
My squad: Samara and Grunt (Both Lived)
Final Squad: Miranda and Mordin (Both Lived)
Remainder Squad Leader: I wasn't given a choice! Must've been a glitch or something. (Thane was the one on the comm. at the end. Very strange)
No random kills.
0 dead total

I put Legion into vents and Garrus as a sqad team leader. And I picked Miranda and Zaeed to my squad.
Then I sent Tali back to the ship with crew (which whole survived).
And I picked Samara to shield my team from bug, and had Miranda and Zaeed again.
Garrus again as other leader and it was fine.
Noone got killed and I saved whole crew
