Jack - She just appeared dead on the floor, i assumed she got overwhelmed.
Grunt - took away by those swarms... dunno how, hes a frigging BRUTE!
and Legion died - Hit by Harbingers missile in the face... Shame, i upgraded his shields and his weapons.

Everyone survived the controlled crash landing:
Part 1: Tali on vents, Garrus as alternate squadleader
Part 2: Jacob on crew escort, Jack on barrier, Miranda as alternate
Part 3: Miranda on rearguard, took Mordin & Samara
All squadmates survived, I'm not sure about the crew but I think the named chars are present.

That's weird... I lost Legion in the vents and Mordin on the second fire team, but everyone was upgraded to the max.
You would think these decisions make perfect sense. Legion is the best hacker in the game, has a familiarity with Reaper technology and is a completely fearless machine immune to pain or organic injury. But he still died.
Mordin led a salarian STG team on two successful suicide missions into Tuchanka, and several more recon missions before and after that, yet he dies on the fire team.
Damn you, arbitrary game class discriminator!

everyone survived, even though Jack and Zaeed weren't loyal and i didn't upgrade my team (the ship was upgraded though)

during the biotic vs swarm part I had assigned him to another task and was surprised he didn't make it
where's the paragon interupt t apply medi-gel when you really need it?!
my first though was that maybe I had taken too long to get through
though it didn't show some bar like with the one in the ducts did it?

-Had all ship upgrades. Only got a few of the upgrades for the teammates. All loyal
-Had Legion do the vents
-Miranda was Fireteam leader both times
-All the crew survived. Had Mordin escort them
-Jack escorted us through the swarm
-Blew up the base
Did all this with Tali and Garrus. No deaths. Then I told the Illusive Man to go **** himself

Vent specialist: A tech expert, more precisely a MECHANICAL tech expert. Some may have failed to note the distinction (Mordin was a DOCTOR) and paid for it. For me, Tali was obvious - Quarians are considered to be the best at mechanical tech among the known races. Some have gotten through with Legion, which isn't surprising on hindsight.
Alternate squad leader: Someone with command/leadership abilities/experience. The correct squaddies are pretty obvious - if you get this wrong you should clobber yourself. My choice of Garrus is a no-brainer - he was ****ing off half of Omega with a squad of ten, which I think is a scary thing to put on a squadleader resume. I switched to Miranda for the second part because her qualifications look good (and one reason I didn't put her first is because Jack raised a stink on it) plus I thought Garrus needed a break.
Biotic shield specialist: Not just any biotic, you want a STRONG biotic. Asari are among the strongest biotic species in the galaxy - Samara. I didn't go with her though - I picked Jack after looking at her dossier and it was a good choice - she was said to be THE most powerful human biotic (created via experimentation).
Crew escort: Ok, this one does not seem obvious. I sent Jacob because he was a former Alliance soldier.

*Doing Jacob's loyalty quest is important, because it shows him what it means to be a good leader.
*Make sure the ship has all 3 major upgrades (Garrus's Thanix Gun, Tali's Oscillating Shields, and Jacob's new Hull Plating)
*Getting the Scanner upgrade, and the Probe capacity upgrade will help in getting the Resources needed to finish the mission.
*get whoever you want to survive Loyal

for my squad i chose tali and thane the whole way through
sent legion into the vents
for both alternate parties garrus was the leader
i chose zaeed to escort the crew back (all lived)
also i had them hold the door. i havent seen any mention of that so idek if you can't have them hold the door but just sayin
had samara lead me through the bugs
yeah so hope this helps someone. i chose garrus because i know he can get the job done and his side mission was about how awful he felt about losing his squad. the only maybe illogical choice was zaeed, but reading his description convinced me
oh and i upgraded the ship totally (except the dermal regen unit because i was planning to be paragon anyway..) and had everyone's loyalty and upgraded everyone's weapons.. my shepard wasn't fully upgraded but i made sure everyone else was ready to go. i also updated everyone's level (as in, used up all their squad points), if that matters. tali almost died at the end in a cutscene where shepard leaped of a ledge to save her. i almost peed myself because he kept missing her hand but finally caught her (she was my li)
ALSO.. i played on veteran, if that has anything to do with it

I didn't get EVERY weapon/armor/prototype upgrade but I got pretty damn close. The ship was fully pimped out, though. Scanning planets = meh.
I had everyone loyal except Legion and Zaeed. Zaeed because I was going full paragon and stopped his revenge, and Legion because I went to the Heretic Station and the Collectors boarded the Normandy, so I high-tailed it to the Omega 4 Relay fully expecting to lose some people. I almost put Legion in the vents, but I decided on my GF (Tali) because she was fully upgraded and outfitted. Nearly pissed my pants when I heard her screaming in the vents. Went Commando on the Harbinger with the Inferno Krogan Shotty to get to the vent in the nick of time. Nearly sharted myself. Kept Tali with me the rest of the way, and now she's sitting comfortably on my couch listening to tunes waiting for more side missions with me and Samara.
I love this game!!!

So for me Legion dies as the tech. Totally bumbed I can't change it. Seems when I have been saving I have been overwriting a preivous save. But I thought a new area always saved a new file, but nope overwrite the old one even with a new name.
Now I guess I have to live with the Loss.

Sent Thane into the vents, ended up dying to a rocket in the face.
Zaeed led the other team here. Team survived.
Samara held the biotic barrier. She and my team survive.
Garrus led the distraction team, looked like he took a shot to the gut, but apparently no harm done. Team Survived.
Jacob led the crew to the normandy. All crew and Jacob survive.
Took tali and grunt with me to the human reaper. Both survived.
It was real sad to see the one casket at the end with Thane in it, I really liked him as a character. On my 2nd playthrough now (which isn't going as fast as I wanted, I guess Vanguard + Hardcore don't mix too well at the start) and hope to get everyone out alive.
What I really would like to be 100% sure on is what affects the outcomes of the choices. My brother chose Tali to go into the vents and she died (he, of course, restarted the mission), while others are reporting she survived. Some report Legion surviving the vent and one or two say he died.

Tech: Legion - he's a freaking machine
fire team leader: Miranda both times - she seems like the most stable out of all the team, and a good leader.
Biotic: Samara - she's an asari, natural biotic, and not insane
Escort: Grunt - who's going to mess with a jacked up krogan? Since it was one escort, it made the most sense to me.
Team: Tali and Thane, until the end boss in which I took Jacob instead of Tali (inferno ammo against collectors ftw)
Destroyed the collector base, told the illusive man to GTFO, and took all the team and normandy with me
Though now I am fearing liara's wrath in ME3. I'll tell her I was just trying to make a dying man happy!

Tech specialist: a loyal Tali or Legion can survive but seem to also die if you don't pick the right 2nd Team Leader
2nd Team Leader (2 times): a loyal Garrus, Jacob, or Miranda seem to do fine
Biotic Guardian: a loyal Samara or Jack will work anyone else will tire towards the end causing, not their death, but the death of one charcter that you have with you at the time
Crew Escort: this one seems to be the easiest decision, unsurprising given how open to interpretation the task is, so far it seems anyone loyal to you will make it back alive but you can no longer use them in your squad.
Some reports of Mordin randomly dieing for no reason leads me to believe maybe the character weapon or tech upgrads may also play a part.

I sent Legion into the vents, (I thought Tali might do a better job but didnt want to risk her because
Garrus lead both secondary "Fire" teams, felt like I trusted him the most and he has been leading a team in the interim years and I felt like other characters didnt really like Miranda even though she was the "obvious" choice. As stated above I feared the worst when he took a hit, but he made it.
Samara provided me the barrier, I felt she was the most powerful at that kind of biotics, Jack is a bit too crazy and unstable, and Miranda isn't as powerful as those two.
Jacob went back with the survivors, he's that kind of guy, and can handle himself and protect civilians.
Grunt and Thane stayed with me throughout.
Everyone survived! I was delighted.
