Sent Legions to the vent - Miranda 2nd team leader - Everything was Fine
I thought I could save the characters I like if I took them with me, so I used Thane as Biotic barrier and the beez came and Took Garrus away... ;__; I was sad.
Chose Miranda as 2nd leader, and Zaeed to escort. All was good and gravy.
Choose Grunt and Thane to fight the reaper human, and everyone else fought by the doors. - Moridin died... gaaaah. not him too!!
So I like my man Garrus and the aweoms Moridin. Good thing I replayed and saved them all.. but I think Imma go back and have Zero die... why not?

Samara died after Human-Reaper, as a result off the fall.
Jack died in the defense off the door.

Well, you need them to be careful with who you choose. I would recommend:
- Miranda as fire squad leader / Legion or Tali as Tech (I chose Legion as I romanced tali, I didn't think I could cope it she died D:)
- Jack/Samara to use biotic sheild (I chose Jack)
- Mordin to go back to Normandy
- Jacob to lead the rest
and I took Grunt and Thane for the finale, and everyone lived. I just had to make sure I had all of their loyalty and upgraded everything/levelled up all of my characters and equipment, but I don't know if that affects the end. Meh. They all lived, I'm happy

Still a blast to play, though. Loved the ending shot of the main story mission cut scene with Shepard looking out at the stars, and then something looking back.

Both times I had everyone loyal, first time almost all technologies, second time I had everything researched.
First time I send Legion through the vents, he gets missiled, second time I send Tali, she gets a head shot. Teamleader was Garrus/Miranda.
Next one I keep on losing is the distraction team...first time I picked Grunt, cause what's a better distraction then a raging Krogan. He got a shot in the belly. Second time I picked Thane, who is dead man walking anyway...shields failed, assassin dead.
In all other scenes I have no problems getting my team through, but the vent and the distraction team leader die and I have no idea what I could do differently about it.
Ps: I had romanced Miranda the second time, but losing Tali really sucked.

Why did you necro this thread when there are so many more recent ones about this topic?

To make sure your team-members survive follow these simple steps.
Upgrading Normandy
You can get these upgrades via the "investigate ---> Normandy Upgrades" from the dialogue wheel
Get the weapon upgrade FROM Garrus
Get the armour upgrade from Jacob
Get the shield upgrade for Tali
This will ensure that atleast 3 members will survive the flight through the Omega 4 Relay and the fight and flight through the debris field towards the Collector Homebase.
Teammembers
Do every loyalty mission and either use the Paragon / Renengade option to settle the peace without losing someone's loyalty. Non-loyal teammembers CAN survive the the suicide mission if they are NOT tasked with an important specialist job.
Tech specialists:
Tali and Legion are the only teach specialists. Really, no other member can surpass their tech skills.
Fire Squardleaders
Garrus - C-sec officer, merc leader (was betrayed but no mention of crap leadership)
Miranda - Lazarus Project Director. She answers only to TIM
Jacob - Former Corsair / Alliance soldier. Head of security at the Lazarus Project and only answers to Miranda (and now to Shepard:))
Note that these 3 are also LI's
Use anyone else as a squadleader and someone WILL die.
Biotic Specialists
The only true biotic specialists in the game are:
Morith
Samara
Jack
Samara and Jack really dont need any explanation.
Morith is only available if you side with her while doing Samara's loyatly mission and kill Samara.
Mordin or XXX dies for no apperant reason
We've seen these threads pop-up quite often. They didnt die for no reason. One of the devs made a post about it.

Mordin's death is not a glitch or bug. I believe that when people are surprised with Mordin's death, it's during the "Hold the Line" time where you leave crew behind as you and two squad mates move onward. Tech had their moment in the sun with the tunnel infiltration. A biotic is key for the protective bubble. The Hold the Line sequence is time for your soldier types to do their best.
Basically you want your best DEFENDERS to hold the line. Don't think about offensive solo strikers. Think about the guys that can dig down and hold a position with moxie. You want them to hold the line.
Under the hood, each character has a "hole the line" score, which gets a bonus if the character is loyal. Characters like Mordin, Jack and Tali are squishy and not exoeruenced with bunkering down and holding out for an extended period of time. Characters like Grunt and Zaeed are tanks who thrive on this type of work. The score is tallied for all the team members that are holding the line, and the number of survivors are calculated. If people are to die, the non-loyals squishies go first through the list to the loyal tanks. Alas, I think Mordin is at the head of the death list. Some characters can't survive without help even if they are loyal. Others can survive even if not loyal.
Here are some examples for the group holding the line:
Loyal Mordin by himself: He can't hold the line by himself. He dies.
Non-loyal Grunt by himself: Grunt lives. He hunkers down and gets the job done, and doesn't have to worry about helping any one else.
Loyal Mordin and non-Loyal Grunt: Grunt dies, but Mordin lives. Grunt is able to hold the line but goofs up helping Mordin... he was just too aggressive without his right of passage.
Loyal Mordin and Loyal Grunt: They both live.
Note it is possible to get non-loyal henchmen to survive through the end game with the proper group holding the line.
So what is a good strategy? Send back a loyal squishy with the crew, like Mordin or Tali. Leave your defensive bad asses to hold the line (hmmm, Garrus defended a base all by himself to an extended period of time....) and take your loyal offensive favorites with you to the end.
As for saving Kelly and the crew, you just need to make sure you go through the Omega-4 relay right after the crew is taken, and make sure you send a loyal squad mate back as an escort.
