But I didn't make the same choices on subsequent playthroughs - what would be the fun in that? E.g. I chose Miranda to put up the biotic bubble - after all she seemed confident she could pull it off. That decision killed Thane. Yet another Shepard chose Jack to the lead the first fire team, just for the lulz (Miranda's reaction was almost, ALMOST, worth Tali's life).

So say somebody puts Tali in the vents, but then puts Zaeed as the leader, Tali dies. Does that make them an idiot for thinking Zaeed can lead a team? He was once leader of the blue suns after all.
Also if the people you take with you to fight the human-reaper aren't
loyal they die... and someone can randomly die towards the end.
Again, does that make them an idiot?
Yes, he was co-leader of the Blue Suns... until his own men & his partner betrayed him many many years ago.
Since then, he has spent most of his time working alone.
If you talk to him, he will tell you that the few times he hasn't worked alone, his whole team tends to die.
So, not really a good choice to lead the second team IMO...
If you take people with you for the end & they die because they are not loyal, then that's your own fault. The game makes it quite clear that loyalty is important - if you rushed ahead to the end without doing all the loyalty missions, then tough, people die.
Deaths during the "Hold.The.Line" part while you're going off to the final part are not random. It depends on who you leave behind, how good they are at defending, whether they are loyal or not. One of the devs explained that you need to leave your best defenders, such as Garrus, Grunt, & Zaeed, to have the best chance of others surviving that section. If you take the tanks with you for the boss, and leave a load of squishies behind for an intense firefight defending the door, then expect to have some deaths...

cool story bro
Its not rocket science. its so easy o have everyone live. You ust have to not be a ****ing idiot and know who is good at what.
Its called getting rid of the team mates you do not like. If you want to keep your team a live thats fine. but gamers like myself who can not stand Tali or Jack and Samara as well Thane have every right to kill them off at the Collector's base.

For example, if unloyal Jacob bites the dust, can I check on his dad after the return from Omega-4?

Jack, Legion, Mordin, Garrus, Jacob. I saved all the crew of the Normandy.

Lost Mordin first two times. First time, I sent him through the Vents, and I took Grunt with me to kill the Reaper. Mordin may not be the perfect choice for the vents, but he's hardly an illogical one, and I didn't know about the 'Hold the Line' value by my second play-through and figured if they were loyal, they'd survive since I didn't loose anyone in the Hold the Line my first time through.
He's not that much of a logical choice though, IMO. OK, I do think he'd be a much better choice than Thane or Jacob, purely because he's a genius, but he is not the kind of ideal Tech Specialist you need to hack the security & open the doors.
The specialist selection screen says "Choose a Tech Specialist to unlock the door."
Tali's
bio says "Revered as a mechanical genius even among the advanced
engineers of the quarians. She is most effective when faced with
technical problems that can be solved with the application of
intelligence rather than brute force."
Legion's bio says: "Utterly
fearless and proficient in combat. It possesses the collective memory
of the entire geth, and has the unmatched technical expertise of a
sentient machine that can be used with calculated proficiency."
Whereas Mordin's entry, although it does mention he's a genius, also specifically points out that his true area of expertise is biology (something players should know anyway from the character). Unlike Tali or Legion's bio, it makes no mention of being a mechanical genius or having unmatched technical expertise etc., which are the things you ideally need for your tech specialist given that they're going to be having to do some fast hacking to open the doors & let you & the other team through ASAP.

As I said, if you took the time to read about them he's clearly not the best choice, but if you don't know that you need to pick one of the best two choices(as I didn't) it's not unreasonable to choose him. Clearly a mistake in hindsight, but whatever.Mister_Tez wrote...

second: Tali, Jacob Miranda. i wanted more to die, but somehow Miranda survived the bullet to the uterus (she was not loyal). she got hers holding the line, though!
third: everyone! it was awesome. well, Legion didn't die, but i didn't activate him in the first place.

Legion died for me, but I am reloading the game to do all the side missions without losing crewmen.
He was my vent fella.
Legion can't die. He's probably uploaded to another Geth server or something..
right? D:
Well his 'body' was in a coffin at the end, but you could have a valid point. I don't see why they didn't just replace his head piece personally. Guess we might find out in ME3
I'm guessing that the speed of death is faster than the time needed to remotely upload a Geth's programs back onto a server, so that depending on the damage done to the Geth's platform, the programs that make up an individual Geth's personality are gone for good.

All upgrades, all loyal.
Squad 1: Garrus
Tech: Legion
Squad 2: Jacob
Biotic: Samara
Escort: Mordin
That's how you do it.
