Is it possible to save everyone and not let anyone die like your party members and crew memebers? Because on my first playthrough most people died =(.
Is it possible?

Yes. You have to have all the Normandy upgrades, have done all the loyalty missions, and choose the right party member for each aspect of the final mission.

I think when you first have the choice to go to the Omega Relay you are allowed to do one side mission and then still make it in time to save your crew.

Crew members: They die if you don't go to the Omega 4 Relay ASAP after they get taken.
Squad members: They die depending on how you place them, if they are loyal and if the normandy is upgraded or not.
