It wasn't dead, they were in the initial stages of building it. EDI states it's in early development, like an embryo. Then it attacks you.

He's referring to the wrecked reaper you get the IFF off of.

When you fire your guns in space, it keeps going till it hits something. Besides, they say its 37 million years old. Who knows what happened.

the illusive man said they tracked to location from a crater impact from a mass driver weapon and found the reaper. so some civilization managed to at least fight off one

in ME 1 you could land on a moon near a planet that had a massive trench carved out of it, popoular belive that it was a huge mass accelerator as the formation is unnatural, using this the illusive man found both the weapon and the reaper, dont know who killed it but cerberus now has that mass driver as well, that does not bode well since i got on his bad side XD

Its hinted at repeatedly if you search and read the planet discussions in the same system as it:

A giant mass effect rail cannon either hit, or grazed it, the shot was fired from an unknown race and planet. The shot continued past the reaper, grazing one other planet(Causing a large ravine) and colliding with another(Likely wiping out its inhabitants)

I can't recall the two planets names, but they existed in ME1 and had similar foot notes in the planet descriptions. The Illusive man notes that this weapon was likely a last act of defiance, as any weapon powerful enough to launch such a power full mass effect field would have to be one time use and incredibly resource intensive.

Edit: Mass accelerator, not mass effect field. Got my biotics mixxed up somehow in that :P
