I thought about going through again so that everyone is alive, but the semi-bitter ending is better in the end as it gives the ending more emotional weight. When playing the game I didn't have any ulterior motives in mind when selecting people for jobs so I think the outcome is genuine. Better that then going back in and trying to fabricate an ending where everyone lives through an apparent suicide mission.
My breakdown:
- All crew loyal
- Legion as the tech spec
- Jacob as team leader one
- Samara as biotic spec
- Grunt as team leader two
- Jacob as crew escort
In the end I lost Grunt (I believe because he either was unsuitable as a leader or I might have missed one of his Krogan Vitality upgrades) and Mordin (not sure about this one, I did only have 3/5 Tech Damage upgrades). Luckily (harsh right?) neither of them was a favorite of mine. I suppose it doesn't matter though, since I'm gonna go through again and I'll prob have everything maxed to the gills at that point.

Although, I wouldn't have minded if Zaeed died, as he meant just enough to me without me actually liking him that he only would've given it more meaning.

No you are right, you can't have it that only Shep dies...
You probably can have it where Shep and 2 others die though. Basically the only way I know for Shep to die is for him to go into the final fight with 2 non-loyal members, whom due to their deaths cause Shep to be running to that ship with just Joker there.

I choosed this.
It was obvious to me that choosing someone to "hold the line" those people would die. But, anyway, I still managed to save everyone in my first playthrought (minus the normandy crew, sorry Dr. Chakwas, couldn't risk to weaken the team), Insanity difficult.... I guess I managed to kill the last boss faster thank's to adrenaline burst (1 burst and I managed to destroy two tanks at once).
Anyway, I felt my Shepard didn't get near his death a single moment... I guess I'll make a terrible playthrough just to see him dying, lol.

Garrus told you he ran a "justic group" type, Miradan was in charge of missions and shepards recovery, & is 2nd in command to shepard aswell, Tali also got leader skills (freedom progress forexample). Rest is pretty obvious for who can do tech and bio. =P
Escort dosnt matter i think, mabye just need to be loyal(?).
To get the crew to die and yourself one pretty much gotta drop loyalty missions and pick wrong team leaders ect. And i think BioWare dropped it just a tad in that area, i mean loyalty missions are the hole game and picking the wrong leaders just dosnt make any sens unless ofc you havnt paided attention to your crews stories xD.
And since Paragon and Renegade is so easy to obtain, you just cant fail a loyalty mission unless you actual direct yourself to do it, heh.
Shepards death should have been a dragon age moment type rather then missing the hole game playing dumb.

I was honestly hoping for some heroic choices in the end about my hero living or dying and instead i found out i gotta be a numnut the hole game if i want to end up dead. =d
I had all upgrades both ship and team and weapons/armor shields everything, every side quest done 100% of milkyway explored, sitting on roughly 200k of each resource (element zero) all characters loyal, full paragon.
I had everything completed before going to get the reaper IFF. After getting the IFF i went and completed Legions loyalty mission. I had spoken to every crew member after everything even if it meant a trip back to the ship and taking longer. I had completed every possible conversation with crew members upto there rinse and repeat eg. grunts says somthing like "i have everything battlemaster" and you cant get anymore out of him. All my crew members were like this except Garrus as my fem shep was romancing him and he just kept asking if i had changed my mind.
All crew members survived the landing.
Sent legion as Spec, maranda as fire team.
Legion got shot in the face and died as he was closing the door.
Reloaded game,
Sent garrus as Fire team, Tali as Spec.
Tali got shot in the face and died and she was closing the door.
Reloaded game.
Send Grunt as fire lead, Jacob as Spec.
Jacob got shot in the face and died as he was closing the door.
Reloaded game.
Sent Tali as fire lead, Mordin as Spec.
Mordin got shot in the face and died as he was closing the door.
Reloaded game.
kept reloading over and over tried every possible team lead, every possible Specialist.
no matter what, the specialist died irriguardless of who it was or who was leading the team.
on my second play through, this time as a male shep with both paragon and renegade low, but same deal, all side quests and loyalties completed. My tech spec all ways dies reguardless of who i send. this time however coz his paragon and renegade were split, i had to pick a side with tali/legion, maranda/jack and subsequently didn't have legion's or marandas loyalaty. reguardless they all survived except for whoever i sent as my Spec.
btw, it doesn't matter who i pick to do anything, team leads, biotic spec anything no matter what i do or say NO-ONE else dies irriguardless of my choices. All crew members of normandy survive, all other team members survive. its ONLY and ALWAYS my Tech Spec as he/she is closing the door.
So it leads me to conclude that, 1, the potential for people to die is decided before leaving for the suicide mission and, 2, i've missed somthing in both my play throughs that causes the tech spec to die.
Gotto be a complete tool to have people die? i dont think so.
Im playing through again, this time on insane as an adept. Hoping i dont miss whatever it is I've missed in the other 2 play throughs. Although im having trouble figuring out what that is exactly.

What I've seen so far....
Do NOT take Samara along during the bubble phase as a squad member. She always dies. Good bubbler, bad squaddie for that part.
Tali ALWAYS seems to get teabagged by a rocket if you send her in the pipe, regardless of upgrades\\loyalty.
Garrus leading the 2nd squad = win.
Thane leading the survivors back to Normandy = win.
Grunt leading the diversion group = FAIL!!

My ship upgrades were 100%. My squad upgrades were at least 30%, most were higher.
Legion did the tech job.
Miranda always lead the Alternate squads.
Jacob escorted the crew back.
Jack made the biotic shield.
=absolutely everyone lived.

From the sound of it I may have gotten lucky though but I'm actually considering having a few people get killed off in a NG+ or something. It just doesn't seem like I made it through a big, freaky suicide mission if everyone makes it.
