Mine was strange. I had everyone loyal including Zaeed. I sent Garus as the Tech and lost him which was odd, his background made him sound perfect for a solo mission. I used Jacob as the Fireteam leader in the first part and he did well. I sent Zaeed with the doctor.

Second part I had Jack escort me and sent Miranda (my LI) with the others. When we got to the door it looked like Miranda got shot but then she said she was ok.

I took Miranda and Thane with me on the final assault and lost Mordin some how.

I really dont understand what I did wrong. My assumption was as long as everyone's status was Loyal and you had shields, armor, and weapon upgrades you would not lose anyone.

Uh, Mordin is ex-STG, and if you did bother talking to him, he does not like "holding the line."

In my last play though I had all ship and nearly all character upgrades. everyone is loyal.
It might have been due to their special unlocked abilities not being upgraded sufficiently.
Went though the Omega-4 relay ASAP after the crew got abducted.

Tubes: garrus, tali, legion. *(all died in my case, but I think its because their stats weren't right )
First mission 2nd team: Samara
I send Zaieed back with the crew.

Biotic shield: Samara (nice biotic shockwave, turning a large barrier into a weapon. )
Second mission diversion team: Miranda (her radio messages sounded like she had no problem at all).
Miranda survived being shot.

Final team was Morin and Miranda, left all my heavy hitters behind.
They both survived the human-reaper (don't know if they can die at this point.)
Everyone got away.

I might go back and try to level legion a bit more hoping he can survive the first section.
I like garrus

I lost Legion, got shot in the face when closing the doors. And Miranda was dead after the boss battle. I didn't have her loyalty after the fight she had with Jack.

I made it through with all squadmates, but the crew didn't make it. I figured Dr. Chakwas wouldn't be missed that much, I guess I'll be drinking alone in ME3...worth it.

Thane and Jack were in my team all the time.



Miranda was the leader on the 2nd team.



Used Legion as enginneer.



Sent Grunt with the freed survivors.



Morinth was used against the swarm.



Had not completed all upgrades but everyone was loyal.

I Lost Mordin  everyone else survived. Everyone was loyal and I had most of the upgrades.

Legion - tech

Garrus - team leader  for both

Samara - biotic

Wrex - save survivors

I think I'll try again tonight and take Mordin with me this time to try and save everyone!

Lost Legion, my loyal tech, and Miranda at the very end who was not loyal (unfortunately after her spat with Jack I didn't have a high enough paragon or Renegade score to access either of the advanced dialog options to change her opinion, even after completing all side quests and exploring all planets to 100%.)

-edit- oh and I didn't lose Zaeed even though I didn't have his loyalty.

I lost Legion and Thane. Legion wasn't so bad because I didn't really get to know him, but losing Thane made me want to die.

I had all the ship upgrades, and everyone loyal except for Miranda (who was before the Jack incident)



Right so my team went as follows:-



Legion (tunnel)



Garrus (fireteam)



Jack (bio)



Mordin (escort)



Garrus (fireteam again)



Hope this helps someone.



My team consisted of Jack and Zaeed (1st)



then Legion and Zaeed (2nd)



And i chose to blow up the base (this seemed to make the difference too)

Lost no one my second time, Tali my first (but I reloaded that. I don't usually, but for Tali...)

To go clean:

Make sure you have all ship upgrades. Make sure you have all loyalty. Send Tali or legion into the ducts. Use Jacob, Miranda or Garrus as squad leader. As your biotic specialist use Samara or Jack (maybe Miranda, not sure.)

I found out the hard way that Zaeed is not considered a good squad leader. No idea why not, given he started the Blue Suns and has more combat experience than the rest of the squad combined (well, minus Samara.)

Mordin seems to die sometimes if he's in the other squad at the end.  Either send him back with the crew, or put him in your final squad.  (I had him in my final squad.  Incinerate ftw.)

My plan was to do the second playthrough without worrying so much the little stuff. But after reading how many of us lost squadmates and crew without knowing exactly why, its got me wondering how the hell things are going to turn out.

Even though I lost no one on my first run, Im already worrying over the team for my second.

Kudos to Bioware, you're really putting the screws to me lol.

I had full loyalty with everyone, I lost people mostly because I listened to the strategy guide that said that the only thing that really mattered was loyalty and not talents. So, I pretty much just distributed who I could afford to lose to various jobs. It worked out alright, Jacob and Jack are gone, and they won't really be missed. In fact have to say I'm glad I won't see Jack in ME3, Jacob I could go either way with.

There are upgrades for ship, upgrades for team and a loyality standings, did you do all the loyality missions?

i have all upgrades for the ship and for the crew and everyones Loyality missions are done. the only thing i can think of why i lose my tech in the vents is cause of the fight with tali and Legion i sided with Tali but went to talk to Legion and got him loyal again maybe thats it as the only person i lost was who i send into the vents

Mordin.  The US CE guide says it doesn't matter who you send as long as you have loyalty.  Which is apparently not true.  

I'm going to replay the ending anyways, so before I do the relay I'm going to gather more minerals and do more sidequests.  Maybe that'll help.  

It's probably been answered already, but you have to make sure the fireteam leader is loyal too, or the specialist will die. I'm also pretty sure the specialist needs to be good with...tech. Legion or Tali.

I lost Legion (tech specialist) and Thane (leader of the second squad), which really sucks because I loved those characters. Anyway, I'm starting my second playthrough and hope to get everyone out alive...

First time I played: lost crew, Chakwas, Zaeed and Jacob.

Second time: no one

1st playthrough, only lost Jack because i didn't upgrade armor on my ship.
