1st Squad Leader: Garrus (has leadership experience)
2nd Squad Leader: Miranda (has leadership experience)
Shield Barrier: Jack (is powerful biotic)
Escort: Grunt (is tough as nails)
Add those choices to the list, because they are confirmed to work. 38 Hours Play Time. Level 25
All team member recruits were loyal. Normandy had all upgrades. Most of the biotic and tech powers were upgraded. I finished all loyalty and side missions before I went to get the IFF, Legion and his Loyalty quest, before going through the Omega 4 relay. I immediately went to rescue the crew as soon as the option was available. My Shepard was imported from ME1 and a maxxed out paragon with some renegade. Personal and party Shields, Armor, and Weapons were all upgraded. My Shepard was a soldier using the assault rifle/machine gun upgrade, upgraded sniper rifle, and collector weapon 99% of the time. I never bothered controlling the other party members during combat other than to tell them to move to a position or move to cover. I just let them do their thing and if they went down (which wasn't often) I used the upgraded unity to get them back on their feet.
I took Mordin and Miranda with me as the final party choice (use unity to keep them alive during this fight. If you don't and they go down I think they wind up dead in the end). Everyone lived except an abducted non-party unknown (at least to me) colonist or abducted non-main Normandy crewman. I couldn't tell who it was, but I did receive the achievment for getting everyone out alive, so go figure.
P.S. I logged into a "post ending" save to check the ship crew and found that every non-party crewman who had a name and you could interact with was still alive.

Tech Specialist: Legion

Phase Two
Crew Escort: Grunt

My Team: Miranda and Zaeed (his grenade is awesome against the husks)
Everyone, even the crew survived when I played it like this. I first tried picking Grunt to lead the 2nd squad during Phase 2 but he didn't make it with someone escorting the crew - don't remember who but they all lived. I started over and let Zaeed lead the 2nd squad with Grunt escorting the Normandy crew back. Zaeed died but the crew made it back.
My third attempt had no one escorting the crew and Thane leading the 2nd squad during Phase 2 but both the crew and Thane died.
Finally I picked Garrus to lead the 2nd squad both times, had Grunt escort the Normandy crew back, and blew up the base. Everyone lived. Hurrah.

At least he went out in a blaze of glory.
THANE was your tech guy? No wonder he died, he's just a biotic with a sniper rifle!

All teammates were loyal. Had all ship upgrades that matter.(heard people lose teammates during ship crash)

Finished the last fight with Miranda and Garrus. Triggered the explosion and as the team is running for safety tali was just laying dead on the floor. Not sure what parameter set that, everyone and everything (armor, weapons, ect) were full upgraded everyone but Zaheed was loyal. Just a random senseless death. Even Moridin made it through the finale and I keep hearing he's a bullet sponge

Also, putting Mordin on escort seems to keep him from randomly dying.
A BioWare staff member on the forums confirmed that Legion can survive even without his loyalty quest. He said that "you can do his loyalty quest after the final mission". Don't have a link though.

Final missions were Grunt/Legion and then Mordin/Miranda
All were loyal, biotics/tech fully upgraded. All Normandy upgrades, Most weapons/armor/shields upgraded. Mordin's omni-tool upgraded, Legions Widowmaker, and Grunt's Claymore Shotgun. Saved the collector station and destroyed it, everybody lived both times.

Mordin was the only other who had died while "holding the line".

I went with..
Garrus as team leader every time.
Tali in the vents.
Jacob to escort the crew back to the ship.
Samara to get us through the swarms.
For the final battle I brought along Legion and Samara. Also my friend Cain joined us.
All of the loyalty missions were completed. I was full Paragon, and I went ahead and researched every upgrade I could find.

I didn't even realize you could save the entire crew, I only just finished my first playthrough. I acquired Legion, activated him, did his loyalty mission and then my crew got snagged by the Collectors. Before all of that all members were loyal, with the exception of Miranda who was 'normal' thanks to the stupid catfight btwn Jack and her, and I did all of the ship upgrades etc.
After my crew was kidnapped I went to a few different systems b/c I was paranoid about weapons upgrades, I did not complete any additional missions...
Not all of my crew survived ..by the time I got to the Collector ship Dr. Chakwas explains how she watched other crew members dissolve (soooo sad and revolting, truly horrific).
Apparently shopping doesn't bode well for the Normandy crew once the Collectors storm your ship..so definitely avoid that! LOL. :\\

There seem to be three levels of this:
1. You are congratulated for coming so quick.
2. You are asked why you didn't come sooner (partial crew death)
3. My assumption is if you do a bunch of side quests that all of them can die?
-Garrus or Miranda for Team leader 1.
I used Zaeed, and he survived.
-Legion or Tali for the Tech Specialist.
There's some other factor here, because I know first hand they can both die in this role, as well as that they can both survive - even when loyal. Legion definitely died when Loyal in this role. Though it may not have been intended, there could be a bug?
-Escourting crew members back - (I've only chose Mordin or Thane so far, and they've both survived. You may have room to play around with this character. As in, if you do not have a loyal member, this may be a free card to save them. I have not tested that method yet, as both Mordin and Thane were loyal.)
I can say that Zaeed, Grunt and Thane all work for escorting crew members back by personal experience.
-Samara as Biotic escourt through the swarms. (I've seen other's post using Jack and it being successful. I assume Morinth is also adequate with biotics. I have not tested Morinth yet.)
Jack works very well in this role. She powers through it. Morinth works just as well as Samara. Jack is the most powerful Human Biotic you could ask for, no matter Miranda's claims. Miranda doesn't survive if you put her in this role.
-Your team members to fight the last boss must be loyal to survive.
They really give you a scare anyways though.
To survive the mission yourself, I'm not fully clear on the adequate steps to take. I've had my femshep fully leveled on her job and had Normandy upgrades and survived.
I've gone through bellow max level and had her survive. I don't think it's connected to that. I have to assume that some of the muscle upgrades may factor in . . . but truthfully? I've never had Shepard die here. Morinth accounts for my only non-combat Shepard-Death.

all of mine have been confirmed I believe but I will post anyway
Tech=Legion
Leader 1 = Garrus
Biotic = Jack
Escort = Jacob
Leader 2 = Garrus
took Mordin and Grunt with me both loyal and survived. Grunt was falling offl and I had to grab an pull up, must have been tough for my Shepard.

For the Tech I picked Legion.
First leader was Miranda.
Biotic was Jack,
Escort was Jacob.
Leader number two was Garrus, he got shot but it didn't phase him (tough son of a **** as Shepard once said, or something along those lines...)
Final team was Legion and Miranda with no one dying there.
Gonna retry Samara's loyalty mission and see how that turns out.
Edit: Also is it possible to have an ending where only Shepard (and Joker) survive? I want my sole survivor Shep to relive Akuze.

Tech: Legion
First Leader: Jacob
I got to all of the valves in time and they both lived. Did too many side quests so most of the crew died.
Biotic: Samara
Second Leader: Thane
Escort: Tali
Samara was able to hold the shield although at the end it was wearing her out a little. Thane gets a gut shot wound while the door is closing and dies. Tali makes it back to the Normandy with survivors ok.
Final Team was Garrus and Jacob, both lived.
I've been reading some posts and I have 2 theories
1. is that your specialist and fire team leaders affect the other as to which one dies (i.e. first leader affects first specialist and second specialist can affect second team leader)
2. is that certain people no matter what will not survive as team leaders or specialists (I really didn't understand why Thane died, he was one of my regular characters, loyal, and fully leveled)
I'm also fairly certain that no matter who you send as escort, as long as their loyal they will survive.
Anyways let me know what you think.

I used Grunt as the escort, chose to keep the Collector base, and Legion survived the fight. If Legion is dying, then it's because of bad team assignments, disloyalty, or failure to upgrade -- I did build his sniper rifle so maybe that had something to do with it.
For my ME3 import playthrough, I'm destroying the base. I wanted to see the "renegade" ending, but you'd have to be a moron to trust the Illusive Man with that base -- hell, Cerberus already proved they can't handle Collector tech.
