hmm, according to this guide, all should have survived.
I did all loyalty quests, got all ship upgrades.
I decided to send Legion through the vents, and selected Garrus as the leader for squad 2 (as he used to be a C-Sec officer, and i didn't want a Cerberus-person doing that).
at the first regroup, Legion took a bullet in the "head", and was my only casualty.

I've never heard this happening before.  Are you positive Legion and Garrus were loyal?  Legion must be loyal to survive.

Here's my choices, and everyone survived:

Team 1 & 2: Garrus (Natural leader)

Tech: Legion (Obvious choice)

Biotic: Jack (Friggin' love the part when she lets the final wave go!)

Final battle: Mordin & Miranda (Used them through entire game. Why stop now? LOL!)

Is Mordin's death entirely random or something? The only disloyal party member I had was Zaeed, who got squished by debris, but Mordin just died out of nowhere.

The really important thing to remember is NOT to do the IFF until you have done everyone's loyality mission (Legion the exeption because that is where you pick him up).  After you complete the IFF you'll need to talk to Legion. Unlock his loyality mission and then you can do his mission before the Normandy is attacked.

The second thing is doing Jack and Zaeerd Loyalites last before the IFF mission.  You need to have done almost all the possible conversation and piror mission to get your Paragon or Renegade level high enough to convince them to stay loyal.  Don't count on second chances with Mirinada or Jack (possible bug).

Anyone not loyal with the exception of Zaeerd will probably die.

yeah Zaeed I messed up his loyalty mission but he still lived. With Miranda and Jack I know if you tell one to stand down at their fight, you can get loyalty back later with enough paragon or renegade (i think it might have to be 100% though, I used the skill with + para and +ren to do it then just retrained later.

Forgot about Garrus leading a merc group, it was a little interesting as I did most of the game with miranda in tow to Warp + overload and break down whatever different protection types I ran into, I'll post my roster though

tech: Tali (seemed easy choice to me)

alternate leader: Miranda (tough, but she has an ability about leadership so i figured was a safe bet)

shield: Samara (made me nervous at the end but did it)

alternate leader: Miranda

crew escort: Jacob

with me at the end: Garrus & Thane

I went straight to the suicide mission after the crew was kidnapped.  I managed to save everyone except Thane.  He was the one I sent through the ventilation shaft.  I sent Tali back to the Normandy as the escort for the crew and they all made it.  I took Grunt and Legion with me for the final boss battle and they performed really well.  Didn't have to tell them where to stand or use unity once.

Per the Prima guide, pg. 219:

"There is a chance the leader of the second fire team is shot down just as the doors close."

This suggests to me that it is a purely random event, and hence survival is not assured even if you do everything right.

Does doing loyalty missions actually matter apart from who who selects to save the crew?

Also I went with Tali in the Tali/Legion argument and both survived.

A lil sidetracked but how do you get Morinth??

I've yet to test this, but I believe you must be renegade to automatically take her side or possibly choose.  I was high lvl paragon and didn't get a decision.  I automatically chose to support Samara.

I think it's dependent upon whether you can successfully resist Morinth's advances; I had enough paragon points to choose the paragon option so I did so, and I got to choose.

Morinth works as the Biotic.

She's basically the same as Samara every where but in her room.

I think, no matter how loyal they are, how many upgrades you have and how "perfect" you assign the positions, there is alway an element of chance wether someone dies or not. Pretty stupid, imo.

Hm, well, as it so happens, when I play RPGs like this (especially Bioware), I tend to save the 'real' missions for last.  Which means I end up visiting every little potentially insignificant planet/place before doing labeled missions like 'Recruit the Assassin.'  In fact, I discovered that, the recruiting is what sets/pushes the plot forward.  In my experience, that's just how RPGs work.  Therefore, since I held off on the recruit missions till the last moment, I was all upgraded/prepared by the time I needed to retrieve the IFF, already having had all members (minus Legion) plus their loyalty.  After I retrieved Legion, I had one more upgrade to buy on Illium, did so, then proceeded to Legion's loyalty mission and right after, the Suicide Mission.

This is how I had EVERYONE, including the nonessential crewmembers survive.  I found that having the main team members survive was relatively easy.  In addition to choosing the right specialist/leader for the job, I found that the speeches I gave as well had some form of impact.  For example, before the final battle against the human Reaper, your Shepard has the opportunity to give a speech.  I, being the goody goody Paragon, started off with whatever the top option was, followed by 'It's them or us.'  During which, you'll see a couple of people (Grunt, Samara, Mordin) being revved up/nodding their heads in agreement.  Even when choosing the same ppl, I noticed that this option saved some members.

Another thing I noticed is who to leave behind and take with you on the final fight.  I found that taking either Garrus or Grunt sometimes resulted in some team member dying.

In terms of picking who works and assuming everyone is loyal of course, I did the following:

First specialist:  Tali or Legion
First leader:  Miranda or Garrus

Second specialist:  Samara, Jack, or Morinth (yes, Morinth works; I tried it out)
Second specialist:  Miranda or Garrus

I always sent Legion back to be the escort because his tech stuff was just useless to me (lol) but figured he'd be more of a fighter than Tali.

I want to make one more note.  I mentioned that I had delayed doing major missions.  Don't mistake that with being able to hold off on everything and trying to do it all before the IFF mission.  You have to delay the recruitment missions.  Those are the big ones.  Even if you get everything ready before the IFF mission and then proceed directly to the Omega Relay, part of your crew will still die if you proceeded with those recruitment missions.

I think, if you're having trouble with some team members still dying, I've found that the ones that tend to die are usually Mordin or Jack.  So you could always just bring them with you on your final battle.

Yup.  That's my input.  Who knows how much is coincidence and whatnot, but I found this worked every time.

I don't think you have to go right away to the relay. However, I took my time and lost three people. If anyone has taken their time and had everyone live, please let me know.

For the crew you must go straight to the relay to keep them alive.  Your team members are a different story.

Straight to the relay is not exactly true, just don`t land on any more planets, perhaps except for Legion's Loyalty.

You can still mine for resources and get upgrades FROM YOUR SHIP, and come out just fine.

My game might have been bugged, though, since I was able to do Legion's mission BEFORE the crew was captured.

2.) After the Collector's kidnap your crew go straight to the Omega Relay and start the suicide mission.  Do not do any sidequests or travel - this will cause crew members to die for taking too much time.

I read somewhere that you can do two side missions before going through the Omega 4 Relay to rescue your Crew, is that at all accurate?

All team members were loyal, full Paragon bar.

Tech - Tali

Team One - Garrus

Biotic Walk - Miranda

Team Two - Jacob

Final Assault Team - Grunt and Samara

Thane Krios was killed, right when the team made it to the door at the end of the Biotic Walk.

Don't know why though, will replay the final level trying different combinations.

Legion was my tech specialist and he ate it on the first playthru...

Jack was my biotic powerhouse and she let garrus get eaten on another playthru...

Thane was a 2nd team leader and he got shot to death in that same playthru...

the escort has NEVER died or not gotten the crew (what remains of them anyway) back.

Next time I'm rethinking the tech and leader roles and it's samara all the way for biotic support.

Ditto on not trusting Illusive man with that base. It's like trading one evil force for another. Blow that thing away!

*UPDATE*

Just played thru again, nobody died, crew or team. Full Paragon Soldier, Almost every upgrade (was missing like 1 pistol and one SMG dmg upgrade) researched, fully developed character interactions ( I believe this has some bearing) and all loyalty.

Legion was the vent crawler. The dude is synthetic, has no fear, and needs no air. No brainer. :P

Garrus was Team leader one. He didn't let me down.

Garrus was also Team leader two. He shined in that role as well.

Samara was the Biotic shield maiden. She did admirably.  Garrus took a shot to the gut but lived thru it.

The final battle this time through saw me taking Zaeed and Tali. Mordin was the escort, made it with 0 casualties.

I agree with the user to stated that the 'final speech' affects the morale of the team. I also agree with the user who wondered if the speed at which you make it through the final mission affects their life chances. I.E.- I tiptoed through it and took my time lining up my shots, and someone died. RESET. Run through it, power through the enemies, slow down for NOTHING, and they live. I tested the ship upgrade theory... confirmed. People WILL die if the ship is not fully upgraded (by fully I mean for combat, the probe and fuel upgrades don't matter one way or the other). Still blew up the base.

Next time through, full renegade, different romance option, and I'll save the base. Hope they live this time too. :P

I lost nearly everyone on my first try and Jak,Thane and Legion died before we even landed on the collector shuttle

Garrus makes a good team leader for both the other fireteams

I lost Mordin somehow. All crew member was loyal and the assignments worked in the previous playthrougs.

I used Tali as tech, Garrus as team leader and Jack as the biotic who maintains the bubble.

The only difference was that I picked Grunt and Zaeed for killing the human-reaper and not Jackob and Miranda as I did before.
