Remodeling it into a health facility is the very Paragon thing to do.
But you will have to tear it down from the top to remodel those walls for mirrors.

Dialog options on the top of the wheel are Paragon, bottom are Renegade, no exceptions.
Though in my opinion, if you're thinking about it that way, you're not playing the game right. You should do what feels right for you or your character in the context of the story. Don't just pick options because you're afraid of an orange bar filling up a tiny bit more.
