Basically on my very first playthrough, nobody on my crew died. So I figured I'd play through again with a 'bad' ending and see what it was like. I didn't get a single upgrade, and every squad mate was unloyal. I was trying as best I could to fail.
To my surprise, I still made it out alive. Albeit, It was only me, Samara, and Jacob left standing, but I made it out just the same XD
Jack - Console blew up
Legion - Drive core explosion
Thane - Impaled by an I-Beam
Mordin - Rocket to the face
Garrus - Swarmed
Tali - Shot
Crew - Vanished
Miranda - Shot
Grunt - Failed final boss
Zaeed - Failed final boss
The only difference I can find is if I cut out some squad mates and make it 8 instead of 11. Does anyone have anything better?

It was actually. Seeing my crew mates drop like flies felt really intense, like an actual suicide mission. And by the end of it I was actually cheering for Shepard to make it to the ship.
