I am curious as to if you dont upgrade ANYTHING on the ship, if that is how you die at the end. I know that some characters will die in the attack if nothing is upgraded, maybe the ship cannot fly back after you set the charge and blow everything up?

I tried it once without upgrading the ship. Basically, Jack got lasered, Tali got blown up in engineering, and Thane got stabbed with a beam. The ship still takes off fine though in the end with whoever is left.

I have noticed Mordin always seems to die in the distraction team. You can see him keel over during that clip scene of the group, its a little hard to see. (However, I have had other members fall over during that scene too in the background and survive). Its best just to send him back with the crew.

I'm curious though about the part right after the boss. I have been unable to get anyone to die when they fall off the platform and get debris on them. Does anyone know about these deaths, is it based on a lack of loyalty or actual choices being made? I have always had them loyal.

I just wish it wasnt so restricted but oh well. Thanks for clearing that up

It's not really restrictive you just made bad choices, well sub optimal choices.  For the three critical roles, not including the escort which any Loyal teammate can do, you want to go for the best choices for the best chance of success.

Leaders: Garrus, Jacob, or Miranda.  Others might do it, but you will probably lose the vent infiltrator in the process.

Vent Infiltrator: Legion or Tali are the best fits for this role hands down.  If you want them to survive.

Biotic: Samara or Jack are both the best choices for this part as well, probably Morinth in place of Samara can get the job done.  The Other biotics just don't have enough juice to finish the job.

Final Boss Team: Anyone as long as they are loyal, or they will die at it's conclusion, speculation though.

Escort: Any loyal teammate can fulfill this role.

I will add I am a proponent that there is probably some hidden time element for some of the suicide mission sections but I cannot prove this intuition.

I did all of the above and all of my crew were loyal, and Mordin still died for me randomly at the end.  It was actually quite annoying - if I had made a CHOICE that made him die, that's one thing.  But just some random event that killed him (without showing me) really irks me.  I went back and did it again with slightly different choices (but still "correct" ones) and everyone lived in the end, including Mordin.  This leads me to believe that 1) there is a random chance to it or 2) the final stage is timed.  If it is timed, I'm disappointed they didn't put some sort of radio communication letting you know you need to hurry up.  I know if that were the case I would have gotten my ass moving.

I had the same problem until I put him on the team when I went to fight the final boss.

Send Mordin as the Escort and problem solved.

The choices for roles at the end are pretty cut-and-dry, there's really no room for error if you want to be sure of everyone making it out. Keep in mind though that the entire point of you recruiting this huge team with different skillsets was to utilize them where they are most effective--BioWare seems to be trying to push us to show that we really understand our squadmates' skills and roles in the final choices.

I followed the path of all the loyalty, upgrades. But I was not successful at keeping Legion alive until I made sure I did 2-3 side missions with him in my party not including the loyalty mission. I think its equally important to use each character and make sure their squad points are used. Not a difficult task, you just cant run in half cocked.
