 I saved everyone I could possibly save and everyone survived in the final mission but at the end of the game shepard is looking at a cerberus coffin and I wasn't rewarded the achievement

SPOILERS BELOW:

If there is a Cerberus Coffin then the achievement won't unlock. You need to get to the Omega 4 Relay as fast as you finish the Reaper IFF mission and need to be sure that everyone in your squad is loyal to you, Normandy SR2 completely upgraded, Squad Special Upgrades are assigned (usually weapons or special upgrades that need to be created at the Tech Lab). Also sending the wrong people to complete tasks can prove wrong. However some characters can randomly die even though they aren't assigned to be a leader / biotic / tech (ex: Mordin died on my first play- through even though I didn't assign him to anything.) Try creating / loading a save before you touch the Galaxy Map to the Omega 4 Relay and try to keep those that died as your ally when you re-attempt the mission. If you are restarting the game then it might be best spend tons of time talking and upgrading your allies as well as completing their loyalty missions. You should be able to understand who died at the end when you look at those who are still alive. If someone does die then the scene will be 1 or more coffins at the end of the game. If no one dies you will enter the cargo bay brushing off a piece that came off in the hall then walking (seeing everyone) then straight to Jeff (Joker). (Well at least that's how it worked out for me.)

nobody died in my playthrough but he was still looking at a coffin and I'm like wtf

I have the same problem. Mordin died but Everyone is loyal and upgraded. Is there any way that it is glitched? I also had a few oddities in the normal play through. Like stuff yhat was paragon actually coming back as renegade.

idk y but i had tali as the tech and she got a bullet right to the dome and died:( . Y IS THIS

At the end I see no coffin but yet the krogan is dead and he was not assigned to anything except MY first squad opening the valves. so many bugs. :(

I didnt upgrade the normandys shields, legion died when i was flying though the asteroid belt, tali got shot in the head with a bug closing door with shepard.

SPOILERS!!!



I was fairly successful at this. I had all upgrades for Normandy and most weapon, biotic upgrades with the exception of the tech stuff. My only teammate lost was Thane who was my first tech specialist crawling through the vents and he died as we were closing the door. Though I kept him alive as he crawled through the vents. No easy feat on Insanity as the first play through.



Zaeed successfully escorted the crew back to Normandy, Samara held a biotic field to keep out the seeker swarms, and Miranda led all the fire teams.



My squad was Garrus and Mordin and Shepard survived.



It should also be noted that I had NOT completed all dialogue with Samara. I HAD completed all loyalty missions so that plays a big part. Not sure about the timing of mission between the Reaper IFF and Omega 4, as you have to wait anyways while the IFF is installed aboard the ship.

I had all ship,sqaud,armor and weapons upgrades, all planets were scanned and everyones loyalty. Everyone lived but I wasn't awarded the achievement and shep looks at a coffin at the WHO THE HELL could've possibly been in that box

So...
Tried this many times.
-I'm full Paragon
-Everyone is loyal
-I've upgraded everything as much as I can. Everything.

I read on another forum that the key to getting the tech to survive was to rush each firefight and open the vents before the tech gets there (I'm assuming the first doesn't count since the tech is already there by the time you learn about it). I did this, I was thanked by the tech each time the way was clear when they got there.
They still get a rocket in the face.

I've tried Tali, Legion and Thane as techs, they all die.

Whaaaaaat is up?! I wish there was a reliable resource to help me figure this out, 'cause it sure as hell ain't the BioWare mods.

Thanks for any help you might come up with.

Go check your squad status on the personal terminal and see if everyone shows up.

If you choose the wrong character to lead the fire team, then your tech specialist will get killed.

Now, I don't know what team mates to chose for the objectives during the final mission like sending one of them through the vents. But I can help out on how to survive the final mission. Basically, for a successful game finish, you'll need to make all of your team mates loyal to you, (Samara and Jack weren't loyal to me though) and you also need to fully upgrade the Normany by either going to Mordin Solus or the Upgrade Terminal located in the Tech Lab, if you're having problems finding the right resources to give to either of them, for example: Element Zero, just go on Google and search for any of the four resources, there should be guides to all of them.

I had no problem completing the achievement on the first try playing on normal.
I used Legion through the vents. Garrus as 1st leader and 2nd. Mordin as Retun to Normandy Leader. Biotic Barrier - Jack. I didn't have Miranda's (lost), Samara's (didn't get), or Tali's (lost) loyalty. I did have every Normandy Upgrade and squad ugrade. I saved after each part of the mission.
I'm not sure if this was a bug that I got the achievement but Jack died in my squad during the mission so i let myself get killed and restart at the last save and I still got it.
No coffin at all.

The one time I finally unlocked the achievement was this way:

1.DO ALL of the Loyalty Missions & Upgrade ALL parts of the ship

2. Side with Miranda when versus Jack, if you can't get Jack Loyal don't worry too much.

3. Get Jack's Upgrade(s)

4. Talk to everyone until they say they are too "busy" right now to talk (land, ect. to check)

5. Use Tali as Tech, Garrus as Squad Leaders, Samara as Biotic Barrier. For the barrier (or 3rd part) take Miranda and Jack this MAY guarantee their survival. For the firing squads, I took Miranda and Grunt with me, or I used Jack and Miranda (whichever one's make Tali survive for the first one is the important step). Take Miranda and Grunt for the last part. (Your other members survive based on a "defensive" value, and Grunt is an offensive member, and Miranda is just too "squishy" IMO)

6. Pray that Miranda and Jack survive. Usually you can tell if you did it when Thane (or someone says they need a "quick exit", meaning nobody died there from what I could tell) then it is all up to Miranda when your done with Harbinger's (SPOILER ALERT) Reaper. If she gets up and you don't have her die in your arms then wait til the ending cutscene and there shouldn't be any coffins (even if Yeoman died/was processed).

7. If you get the achievement yay! if not then retry again but with a new tech specialist, ect. where anyone died.



Also I did ALL of the Loyalty Missions last after the Reaper IFF and Normandy's Crew were taken, in this order (or near it): Tali, Grunt, rest of squad, Miranda, then Jack's (Do Jack's last/after Miranda's because if you talk to Miranda after hers it may effect her survival rate). Talking to Miranda seemed to keep her alive for some reason though I think it maybe a bug for them both to be loyal. I did get Jack's loyalty after the suicide mission (Paragon Points/Ending).

Got the achievement on the first playthrough on normal. Normandy fully upgraded and everyone loyal.
