When you send a person to escort the crew back to the ship why does his image leave your squad page? Does he become a sacrifice or get killed or something?

Just means you can't select him for the other missions because he is already on one -- he's not available.

If they get killed, then the image in the squad selection screen will turn to bright red to indicate their death.
