So I had gotten every single crew member's loyalty, and done all of the ship upgrades. Surprise surprise, I still lost my tech expert who you send in the ducts at the end of that part. It was Jacob, and as him and Shephard were closing the door he got shot in the head. Is this unavoidable? Is it impossible to save everyone on the first playthough? What did I do wrong?

It is avoidable. Jacob isn't the best tech to use for the vents. I used Legion, and he made it through fine thankfully. During the final briefing Miranda says that Jacob is not the strongest tech guy which shows you either have to go with Tali or Legion.
