After entering the other side of the Omega-4 Relay, during the cinematics someone claimed "They got THANE !" and boom he hit the floor, is this guy dead now, not using again, I need feedback ME2 World

It means he was one of the suicide mission casualties. I'm sorry, but you'll have to replay it again to try and get him to live.

I think it depends on whether you upgraded the Normandy or not. Also doing his loyalty missions raises his chances of surviving. Good luck.

Some of my losses seemed random *Spoilers ahead*
I lost Garrus after he opened the vents, and I lost Mordin and Miranda after holding the platform so I could kill the main Reaver and blow the ship. I had done loyalty missions for all of them, though Miranda got a little pissed after Jack's mission.
I thought I was going to lose Grunt, we went sliding down a platform, but Shep saved him. I was really, really worried too!

i had his loyalty, i should have done legions because i just lost him and those were my two FAV squad members to use and I didnt make a save point before this so I guess im stuck.... sigh

i dont care if id lose zaeed, grunt, or miranda, i wanted legion... i waited SOOO long for him i hope i can replay this misson or are they just GONE??

Yeah if you don't have all the offensive and defensive ship upgrades you can lose crew mates before you even land on the Collector's base.

You lose crew if any of the following are met:
You are missing a crucial ship upgrade (armor, weapons, or shields)
You are lacking loyalty with them and are involved in an important part of the mission (leading a part of the mission) - Need confirmation here
You assign someone a role they are not specialized in. ie. Using Thane in the vent will kill Vent because while he has tech skills, he is not as good as Tali.
You are missing a crucial ship upgrade (armor, weapons, or shields)
You are lacking loyalty with them and are involved in an important part of the mission (leading a part of the mission) - Need confirmation here

You didn't upgrade the Normandy. You can lose up to FOUR characters if you don't upgrade the ship, that is even before the battle!

It's generally inadvisable to leave him to hold the doors while you go on to fight the Reaper. Otherwise perfect runs usually end up with Mordin killed because he's just not durable enough to last a sustained firefight. A wise course of action would be to send him with your crew back to the Normandy or (to be 100% certain) take him with you to fight the boss.

I can't understand why so many people are letting characters die, except for those the really wanted everyone to. It's so easy to keep them all alive.

Interestingly enough I only lost Legion and I took Miranda and Tali with me on the final part, I've been hearing alot of people talk about Mordin dying but didn't experience it for myself

The losses are really weird, I don't get the system.
In the Collector base I made Grunt the 2nd Squad Leader the second time the decision came up and he did in a video sequence, couldn't do anything about it. I had finished his storyline, researched a couple pieces of krogan armor/weapon stuff, he was loyal etc. but he did. No clue what I could do differently.

Don't make him 2nd Squad Leader.
Characters start dying if you put them in bad positions, for example - Jacob should not be the one holding the barrier against the swarms, he isn't an expert biotic or.. Garrus shouldn't be in the tubes, a tech specialist should.

One important thing to know is even if you do Thanes mission it doesn't mean he'll be 100% loyal.
Thanes can go a few way, if you fail stopping his son at the end of the mission it will say "he is still distracted but willing to go". Also make sure to have all the upgrades for the Normandy.
I'm pretty sure most are like that you can do there loyalty mission but you have to do it right.

Grunt isn't a leader, he's a... well grunt. You need people who know how to lead into combat to make it through that part.
Here's a question... they kept saying Shepard could die, has anyone had that happen yet?
