This issue has occured at least two times here. The first time, I thought I was too tired and remembered it wrong or something, but the second time now I am absolutely sure.

When I did the loyalty-mission for Thane, I went to the Citadel, talked to Bailey and then went up the stairs to find Mouse. I do lots of quicksaving, so I saved before going up the stairs, on the stairs and the moment I was at the upper level I immediately saw him and saved again. Then I talked to him (this might have been before the green saving-symbol disappeared). The conversation didn't quite take the course I intended, so I decided to load that save I had made just a few seconds before talking to him. But after reloading, Mouse was gone. Hell, I thought, gotta restart the quest or something - so I went back to Bailey, but instead of the "Where's Mouse?" I got to ask him where the other guy was (forgot his name). It seems that my savegame actually contained the conversation with Mouse, although I hadn't even talked to him when I saved it.
I don't quite know what to make of this, but it sure is weird and has the potential to really screw with my game one day...
