People are really having trouble with this? I just made the most logical choices and found it to be pretty easy otherwise.
Leader of both Fire teams: Garrus - He's been on the team in ME1 and has had 1-2 years experience leading his own squad. I didn't trust Miranda to keep everyone alive, 'cause she seemed like the kind of person to make sacrifices to complete the mission.
Techie: Legion - This is just a personal choice, either Tali or Legion would be great choices. But Tali was my romantic interest and I sure as Hell wasn't gonna leave her out of my sight, even though I was an engineer and could've used more diversity (made it pretty epic when my Shepard caught her before she slid to her doom).
Crew escort: Jacob - He was my bro and I trusted him to keep everyone safe. He didn't disappoint.
Barrier duty: Samara - C'mon, she's got centuries more experience using biotics than anyone else on the team.
So yeah, base your decisions on experience in the required area. Obviously, lone wolves like Thane and Zaeed wouldn't make good Fire team leaders, nor Grunt 'cause he's what? A few weeks/months old? You basically have to look at every aspect of their past and personality.

i got through the game without loosing any squade members but all my crew members except the doctor died, is it because i waited too long to start the suicide mission?

I... did not make it on the first go.
Legion took a bullet to the face. Grunt had a belly full of.. lazer? Jack was face first on the floor, Mordin was croaked over somewhere...
Second go, I got it. <_<

If I were to hazard a guess, it could be because you left some of the missions regarding the crew undone. Did you do the assignment to improve the cooks food? The one to get those power converter things for the engineers?
Otherwise if you did everything you could to prepare both ship and crew, it must've been your decisions during the suicide mission.

I got the manifolds for the enginers, food for the cooks, and the brandy for the doctors, but the only things i can think of that i did wrong was not go after them imediatly after they are taken, i think dr chakwas does say "why did you take so long" and shepard replies "i still needed to build the team" or something along those lines.

I think its silly, if you choose someone whos obviously going to be a bad leader they shouldn't necessarily be the one who dies. They should possibly get someone else killed. How does your leadership ability affect whether you get shot in the stomach or not?
Also would have been cool if bioware had stuck a variable in where if Miranda where chosen to lead she would make a decision that would jeopardise anothers life to put the mission first.

You lose people for taking too long to go after your crew? OK... then it was completely by accident that my crew all survived... definitely making a note of that!

made it through. Miranda was fire team 1. Legion went in the vents. Garrus was fire team 2. Jacob took the crew back. Samara was my biotic. Miranda & Garrus beat the game with me. & everyone lived happily ever after.

Why is everyone acting like because you were some team picking genius everyone made it on your crew and it is so simple. There is something else going on that just ended up in your favor somehow.
Try this one on for size. Everyone loyal except for Zaeed. Did every side quest and explored every star system including the new ones you can buy maps for, before starting IFF mission. Had every ship upgrade. Had basically every team and Shep upgrade I could afford (which means maybe only a couple things at 4 of 5 power). Destroyed Heretics base with permission from Legion to go ahead with it.
Sent legion through the tunnel. He should be the KING of tech. It's so easy right, NOPE he's dead at the door. My unloyal clown Zaeed still lives along with everyone else. I did lose half the crew in pods I guess because I said hold on a sec in the meeting to check and see if we were ready by making another pass through the crew to talk to em.
There's more to it than picking the right guys for the job. Someone said something about the timer countdown on the switches. I guess that could be it, but a couple of them for me barely ticked down at all before I activated them. I had Samara leading the other team as who could be more experianced than her, Miranda commented on what an excellent choice it was. Though I remember some radio chatter about them having a tough time, but all on her team survived.

I sent Tali into the vents and she took a rocket to the head while holding the door. I had full loyalty with everyone, and all upgrades except one tech damage, one shotgun damage, and one AR damage rank.

I made it through with every squad member alive other than Legion my first time (I chose not to activate) I just beat the game again and lost 4 members
Does difficulty impact a characters survival chance on the suicide mission? First time through was on normal, last time was hardcore.

Legion: Tech Specialist. i thought about Tali, but she couldn't fight off a puppy by hereself.
Garrus: First Team Leader
Jack:Biotic Escort (obvious)
Grunt: Return Escort. He is huge and badass, if anyone could get my crew back alive i thought it would be him.
Miranda: Second team leader. She got shot in the stomach but was fine.
Tali and Miranda: Final boss partners.
No casualties.
It all comes down to just common sense choices. you are given sooo much canon about each character. Grunt is not going to be a good leader, Zaeed is a ruthless leader. Garrus however is brutally just, and was incredibly torn up about losing his squad from Omega, so i assumed he would pull it off flawlessly and he did.
Miranda kept complaining for a leading role so i let her...and she pulled it off as well.
Just PAY ATTENTION to each character...and think about the choices and roles that have to be played and you will be fine.
I think random characters dying at the end must have something to do with the previous personnel choices.
By no means is this the best series of choices, just what i thought would be. make your own path and see how it turns out!

i made it through first time with no outside help...lost 2 of my team though. miranda, who i expected cause she was unloyal due to fight with jack, and mordin. mordin was loyal though, and i see some people saying miranda survived even while being unloyal....
how in the world does an unloyal one survive and a loyal die? is it random or what? btw jacob lead my second team and i had garrus and grunt with me in the bridge assault when everyone else stayed back
