Adds +5% of all elemental types of damage, yes? To which attacks? The default staff attacks? Every spell you cast? Does it work for melee classes (if, say, I put it on my rogue)? I haven't noticed it boost the damage of any attack so I was curious.

if i had to guess id say its for melee? i mean my "war mage" aka arcane warrior is running around with a shield and sword. chopping threw the enemy line in melee and using magic to heal and petrify enemys

You made a Arcane Warrior? How does it do? What diffucult are you playing? Where do you get arcane warrior?

All relevant damage, except that added by a flame or ice weapons spells. I think the additional damage done by those spells derives from the caster's spellpower and items. I'd really like to have a dev answer to this because it does affect how I distribute my stat-boosting items.

My belief is that the +5% on elemental damage applies only to attacks based on that type of element. That is, if you cast a cold spell, it will gain a +5% damage bonus for a cold attack. Same with fire, etc. It won't create a new elemental damage type, like the runes that add specific damage to melee weapons. It will only augment magic elemental damage of the type being cast by a mage.

ETA: In other words, it's a mage item for augmenting damage to elemental spells.  It won't do a warrior or a rogue any good.  That's my experience.
