How did members of your squad die?

1. I sent Tali into the pipe, she lived.
2. I had Miranda lead the second fireteam, she lived.
3. Samara for the swarms, everybody lived.
4. Garrus led the survivors back to the Normady, he lived.
The only one who died was Zaeed, and it didn't really show it, just had him lying on the ground after the final battle.

I had all of these too and I was fine.
Except I am yet to download the Zaeed content, so I had Jacob lead the crew. Mostly because I didnt give a crap about him.
You just have to make sure that you get their Roles right, I was even worried about putting Jack as my Biotics Barrier just cos she is pretty emotionally unstable.

Second time I was a bit more casual in my approach. Didn't get EVERY upgrade i could find and lost the loyalty of a couple teammates.
Only Shepard made it. It was VERY bad. downright disturbing.

During flight battle:
1. Legion can die by being blown up by the core, and does not escape.
2. Thane can die by a piece of hull slamming him into a wall, miranda comments on his death.
3. Jack can be burnt by the ocular laser and Miranda says its too late to get her
4. Tali can be vaporized by the kinetic barrie where she works in the engineering. Likely that her loyalty mission directly affects this, but not certain.
( It is assumed the ship upgrades are the determinign factor, with loyalty not playing a part in this case. Which upgardes are need or how many not determined.)
These 3 are confirmed to be able to die before the landing, although i saw the video where Tali also was blacked out before starting the mission. odd.
During mission:
1. Ventilation member: Survival depends on who is leading the squad, and if they are not a capable squad leader, the ventilation specialist makes it out of the vents, but is blasted by a rocket. No dialogue. Shepard kneels down. Although its uncertain whether the pipe member must be a good tech specialist, as the only confirmed is the team leader must be capable.
2. Biotic mission: A character can be killed during the biotic mission as well. One confirmed reason is the biotic ability, jack and Samara are the only confirmed to be able to keep whole squad alive during it. That is not fool proof though, there is another factor as video shows Samara can also weaken and have a member die.
Note* Member who dies is not the biotic wielder, but one of the two members you choose for partner, when you reach the door, the biotic weakens and a team member is carried away by the swarm, and also screams. Determining factor for which member gets carried away unknown.
3. Door jam: After the biotic mission, you rush to the door to let the distraction team in. In this scenario, only the team captain has been shown to die, and this depends on his leadership and tactics ability ( ie. Mordin and zaeed can be killed if leading team.) No other member is shown to die during this so far. This scenario has the squad mate talking, mordin asking shepard to tell the he " held the line" and zaeed sayign something about he saw it ending this way.
4. You then leave the remaining team and battle the reaper. Once returning, it has been confirmed that a member of the team can die. Mordin is often referenced as dying here, but others confirmed as well .So far, only one member has been shown dead at that time, not multiple members.
5. When the platform crash lands, both of your squad mates who were with you can die. This is confirmed, Shepard will get up and walk over to the first, who he rolls over and they do not respond.The second member will struggle to get the wreckage off, and after shepard picks it up, they die in his arms ( Grunt and miranda confirmed through this.)
6. The final death confirmed is that of Shepard jumping to the Normandy. In this case, no members were left to grab him, so Joker grabs him and can not pull him up. Shepard tells him to leave, and Shepard lets go and falls to his death. In this scenario, Joker talks to the illusive man, and acts out Shepard's role for the ending.
7. Forgot but, the other way a member of the team can die is by escorting the crew back to the ship. Its not yet known why it happens, likely just loyalty, but crew members have died and so has the squad mate as well.

I had done all the quests in the game and had all the upgrades that I didn't miss.
Still I'm surprised Mordin died.

I'll review in chronological order (to the best of my memory) how it went down and what choices I made in each case.
- Got all upgrades for the ship and loyalty for all squadmates before going through. Haven't bothered to unlock Zaeed yet, so not sure how he fits into everything. Completed ALL missions and assignments. Had full Paragon. My character was based on my imported character from ME1.
- Ducts and the first diversion Fire Team:
-- Tali in the ducts will succeed. Tali is able to fix the door so it shuts, when I tried Mordin he couldn't fix it and died trying to manually push it shut.
-- Garrus and Miranda were successful as leaders of the Fire Team. Tried Thane, and I think he was fine, though I'm not certain as I chose not to continue that time and went back to a previous save.
- After finding the Normandy's crew. Chose for someone to escort them back, both Jack and Thane were successful.
- Chose Samara to hold the bugs back and she was successful (it was her idea to use the field, and I figured she has to be the most powerful biotic considering how old she is...).Tried either Garrus or Miranda to lead the second diversion Fire Team while my team and Samara walked through the bugs. Both Miranda or Garrus were successful. Also, in both cases Garrus/Miranda took a bullet to the torso, but weren't hurt bad enough to die and were able to continue.
- I've confirmed Miranda and Legion survived the platform crash after defeating the "people-reaper" (if only it was purple!).
- I've tried both endings in regard to what to do with the station:
-- With the radiation pulse, the star behind the Illusive Man is orange. He is grateful that you saved the technology but the ending is ambiguous about his motives. After talking to him, you can choose to tell him to Shut Up or another choice (I can't remember the wording, something like "I did my best"). In this instance, I chose the "I did my best" choice and I remained a member of Cerberus with the Illusive Man very happy with me. If you continue to play after the ending, you can talk to your squad members and they all express disapproval that you chose to save the station and comment that the Illusive Man can't be trusted with Reaper technology.
-- If you destroy the station, the star behind the illusive man is blue. He is very unhappy and makes it clear he is not sure whether you were a worthwhile investment. In this case I chose "Shut up", Shepard tells the Illusive Man to get stuffed and leaves Cerberus. Talking to crew members after this ending, they are all happy that you chose to destroy the base. Jacob comments that Cerberus will be VERY unhappy with you, and implies that they will be your enemy in the third game.
- After the end of the game, if you choose to continue playing there are a few things you can do:
-- All squad members will tell you what they thought of your choice with regards to destroying the station.
-- You can sleep with your other love interest. In my case, I originally chose Miranda during the game. After the ending of the game, I went to speak to her and she reverted back to our last conversation before the love scene where she says "I think we're crazy, I'll grab you when it's quiet". I told her I was no longer interested. I then went to talk to Tali, told her I was interested and it cut straight to the love scene with Tali.
-- There appears to be a little easter-egg bonus in that after I had slept with both love interests, I got a message at my prviate terminal from Kelly telling me to meet her in my captain's quarters. If you go there, she will do a dance for you in a skimpy outfit (basically the same thing as the Asari dancers found in nightclubs). No lovin' however.
That's all I got. I'm going to play the game through a couple more times to see how being a full Renegade affects the ending, and how things are different if you're not as well prepared as I was.

The swarm death depends on who you pick to do the biotic barrier. Samara and Jack are who you want there because they will unleash a giant repellant wave at the end of the mission, pushing the swarms way back. Thanks Mystranna I guess I kinda figured that Miranda would have been the right choice because she was "made" by her father but now that I think of it Jack took out the heavy mecks with no prob and Samara basiclly put her daughter through a window without trying.

I was expecting an Ashley/Kaidan moment where I PICKED who died so I didn't realize the full consequences of my actions, but I was fortunate. I picked Legion for the tunnel since he seemed like the best person for the hacking job. (Was suspicious that this was a trap so I picked him in case someone had to die and I thought a machine was expendable.) I picked my brother-in-arms, Garrus, to lead the team both times. He had been there with me since day 1 and was with me in the final fight with Sarren/Soverign. There was no one I trusted more than him so he was my first pick always. I picked Jack for the swarm because she was a fierce and determined person. I felt she would give 110% to the job and was rewarded by her expression when she did the final pulse with her biotic powers, pushing them all back before we went through the doors. I had Mordin and Legion with me in the final fight. They were both fine after I pulled them from the wreckage. All squad members had loyalty maxed out.
I was actually wondering what the big deal was with this being called a suicide mission since everyone lived (sans the crew) and didn't realize how far the ramifications could go even with losing the whole team like that. Very nice surprise.

Garrus led the second fire team and lived. Though, he did get shot in the gut in the cut scene but he recovered.
Samara did the barrier and we all lived.
Grunt escorted shipmates back and they lived.
Mordin died holding the line at the end. I was pretty broken up about that since I really enjoyed his character.

I used Garrus 2x, When you first enter the base, and then after you split up again.
Tali in the vents
Jack as the biotic
Zaeed to lead the survivors back (Hoping he would die... i didn't like him) He died.
I took Mordin and Miranda with me to fight the human reaper.
Jacob died at the door i assume, but i didn't like him anyway so he got what he deserved. Unfortunately i was able to keep Jack alive, which sucks. Oh well.

well, I had Jack holding up the field (miranda wasn't with me, Thane and Mordin were, IIRC) I think the main relevant things once you get to the base are loyalties and also who you choose for each task, maybe miranda isn't a powerful enough biotic to make the field so it holds the whole way, I chose Jack based on such reasoning, she's even powerful enough to make it a shockwave at the end and knock the collectors over, dunno if that happens anyway
I had Tali go through the ducts, Garrus lead the first alternate fireteam and Jacob lead the second-Jacob got shot as the doors were closing but was okay

I had 5 squad members die (I had loyalty with them all)
The order they died and what happened:
1. Jack: When the ship was being attacked she fell, hit her face and fell to the ground and died.
2. Legion: He died in the explosion in the engineering area.
3. Thane: He was crused to death by a pipe during the Normandy attack.
4. Tali: She was carried away by the collecter bugs during the mission.
5. Grunt: I sent him with the second team and when I reached the door he said a few things then died.
So many of my people died, I wasn't doing something right I guess.

My ship was upgraded enough so that nobody died onboard. After we crash-landed, I took one team with Miranda and Thane and Garrus took another team with Jack, Grunt, Jacob, Samara, Legion, and Mordin. Tali was my tech specialist, so she went in the ventilation tube. We linked up at the door, and Tali took a shot in the chest and died trying to close it. After that, I had Jack escort Doctor Chakwas and the other crewmember back to the ship, and had Jacob lead the distraction team while Samara provided the barrier for me, Garrus, and Miranda. Jacob took a shot in the gut when we linked up again, but his shields held. Miranda and Garrus went with me to finish the job.
One death, not bad. Going for no deaths this time around.

Just a note, I've never seen the biotic specialist die. The biotic specialist causes one of your personal squadmates to die, but the specialist herself/himself actually survives.
Correct. You need Jack or Samara as the biotic specialist, as they are the two strongest. I chose Miranda the first time simply because my normal group setup was Shepard, Garrus and Samara... Garrus got lolraped by a swarm, however, because Miranda is a sissy who can't keep up a damn shield. I ended up replaying it and had Garrus survive by switching up Miranda and Samara, which made me muuuuch happier.
