I choose tali as the tech expert...I didn't have her loyal, b/c I told her to stand down against Legion. She was shot in the face by a collector, and I don't regret it. Not.One.Bit.

There is an excellent thread around here somewhere that actually explains in detail not just the upgrades you MUST have but who can and should do what job if you want everyone to make it out alive.
Each team member has an upgrade suggestion which is needed if you want them to make it out alive. Jack for instance will ask you for an L5 implant that boosts her biotic abilities ALOT.
Then there is the upgrades to tech, armor, weapons and so on and so forth. You need to be ridiculously meticulous when going through a level cause some of the upgrades can hide in some pretty out of the way places. (I mean there is a upgrade for your shotgun on the wall of Morinth's apartment FFS, which i completely missed my first time there. You can also pick up a ton of stuff in the place where Garrus is holed up doing his Archangel thing ... one again easy to miss.)

If you want everyone to survive do what many people in this thread said. I was able to save everyone.
First for the crew, like explained earlier do not do the IFF Mission till you have all the normandy upgrades and special upgrades for your squad mates. Like the special shotgun for Grunt and the implants for Jack. I also got Mordin his special thing, I didn't get Legion the sniper rifle but he still made it.
For the vent shaft I used Tali, she was loyal and I had previously defused the situation between her and Legion with my high Paragon rating.
I got all the squad members and they were loyal. I also upgraded many things before doing the reaper IFF mission. I knew something bad was gonna happen just because the way EDI was telling me to take the shuttle.
Samara is a natural choice as she is a powerful Biotic to protect against the swarm. To lead the team I picked someone with plenty of experience like Miranda. She made sure everyone survived.
Since I knew Zaaed was a bit older but an experienced merc, I chose him to take the crew out of the collector ship. All of my crew members were rescued. This was because I went right after they were abducted, couldn't leave them behind. Kelly, the DR, engineer, everyone you can save survived as far as I saw. Though some unknown crew member did get liquified.
For the battle I used Legion thanks to his awesome little drone and Mordin for his nasty incinerate. Both survived after killing the final boss. Miranda did appear to take a shot to the gut but she was fine.
After that I saved the base so Cerberus could use it as I know the Reapers are common. I rather have the lesser of two evils than be underpowered for Mass Effect 3. In the end everyone survived, whew.

But then Joker tells me the survivors from the ground team is onboard, and I got confused. No way they could have survived that ;(...

Some things will always happen or will happen if some requirement isn't met on the actual mission itself not counting if you upgrade the Normandy or not.
If I recall, if the techie isn't loyal or not good at tech stuff (or the team leader sucks), he/she will take a rocket to the face because the door gets jammed and they have to force it closed. A loyal techie like Legion or Tali will get it closed without it jamming.
On biotic barrier, if your biotic isn't loyal or not strong enough, he/she isn't able to do that shockwave at the end of that part that pushes the swarm/collectors back and a team mate gets swarmed by the bugs. Jack and Samara are pretty much your only choices as they are the only strong biotics.
If you send someone to defend the crew/colonists you free and they aren't loyal, they die defending the crew/colonists.
The second Team leader always takes a shot to the gut when closing the door. It's a matter of if that team leader is a good leader and their loyalty. If the leader is a bad one or not loyal, he/she gets hit too much before you open the door for him to get in and has no shields left for that gut shot when the door closes.
Then there are the party members you have hold the line to defend your back while you take on the boss. I think party members that aren't loyal or upgraded enough will die.
And finally, there are the 2 party members you bring with you on the final boss. After you beat him, if they aren't loyal, they don't survive the platform crashing down on the ground.
Some stuff may be factored in by a time limit too but we're not sure.

Let Mordin take the crew back to the ship. It is the safest course for keeping him alive. Always let a loyal Garrus lead the second team. Both a loyal Jack or Samara will get your team to the end safely.
A loyal Tali or Legion is needed for the pipe as anyone else eats a face rocket. I send Jacob there to make sure he is not in my ME3.

On the bright side(
), here's how to make EVERYONE survive:
1. Get ALL squadmates' loyalty.
2. Not sure if it matters, but buy the teammate-specific upgrades, just in case it does.
3. Get the armor upgrade from Jacob, the weapon upgrade from Garrus, and the shield upgrade from Tali.
4. After the Reaper IFF mission, you will only have time to do Legion's loyalty mission, so do everything you can possibly think of before doing the IFF mission.
5. Now, where these things start to matter is on the way to the Collector base: if you did not buy the armor upgrade, Jack will die due to an Oculus laser.
6. If you did not buy the shield upgrade, someone will die in engineering. who dies depends on who you took to fight the Oculus.
8. Onto the suicide mission:
Tech specialist: Loyal Tali/Legion. Anyone else, or non-loyal Tali/Legion, will recieve a headshot trying to close the door.
Fire Team Leader: Loyal Garrus/Jacob/Miranda. Anyone else, or non-loyal Garrus/Jacob/Miranda, will result in the tech specialist's death by headshot, even if it is loyal Tali/Legion.
Biotic specialist: Loyal Jack/Samara/Morinth. Anyone else, or non-loyal Jack/Samara/Morinth, will result in the death of your primary squadmate (the one on the left), as the biotic specialist's shield will start failing, the primary squadmate will be carried off by the seeker swarms while trying to hold them off, and the player is forced to listen to their death screams.
Diversion team leader: Same as Fire Team Leader, but if it is anyone else, or non-loyal Garrus/Jacob/Miranda, they will die after taking a shot in the gut as the door closes.
Escort crew back to ship: There are three ways to go about this:

Curiously, only Miranda died in my first playthrough, since she was unloyal (I wasn't paragade enough to settle her dispute with Jack amicably). I actually thought I'd lost more, since Zaeed was also unloyal and I left Mordin to hold the door while taking Grunt with me to take out the Human-Reaper. I actually didn't check to see the actual casualties from my first game till after I'd finished it a second time, and was quite surprised to see that the body count was that low.

So far, I've played the game through twice and both times nobody died. The first time was great because I deliberately avoided these boards and all spoilers so I know I did it on my own.
My reasons for picking each squadmember for the suicide mission in brackets. Everyone is also loyal and all ship upgrades made sure we all safely made it to the base. Plus, they all have their personal upgrades like Mordin's super omnitool and Jack's L5 amps. Also, their specialist classes are all upgraded to level 4 mastery with options increasing their health and survivability - e.g. Mordin's shield was more powerful.
FIRST PLAYTHROUGH:
Male Paragon Shepard Soldier class (my most used power is adrenaline rush).
First fireteam leader: Garrus (due to his experience leading his own team on Omega).
Tech specialist: Legion (due to his being a geth so I thought vents wouldn't be a problem and superior hacking skills).
Two squad with me running to the door and opening vents: Jack and Miranda (Miranda was the LI and tech power member on my squad and Jack provided the biotics powers and cool biatch factor. Plus I wanted to keep an eye on them because of their rivalry).
Result: Everyone survived. You can see while the door is closing and Legion trying to hack it, Garrus ran infront of the door and held off the bad guys.
Escort Normandy crew: Jacob. (He just seemed best suited for the job while also being someone I could live without for the rest of the mission).
Result: Everyone makes it back to the ship alive.
Second Fireteam leader: Garrus (again for the same reasons).
Biotic specialist: Samara (being a powerful justicar and she glows most of the time on the ship showing her awesome biotic control).
Squad: Jack and Miranda (I just kept the same squad because it is a good balance of powers).
Result: Samara successfully keeps everyone alive and has enough power at the end to blast off the swarm while we all escaped. Garrus kept everyone alive again.
Reaper battle squad: Jack and Miranda.
Holding the line: everyone else.
Result: everyone survives. Jack and Miranda get up OK after the platform crashes and everyone else also appears afterward on the ship so I know they all held the line.
That was awesome because it felt like such a big payoff for my choices.
SECOND PLAYTHROUGH - I decided to shake things up more this time!

Escort Normandy crew: Zaeed. (He just seemed badass enough to keep the crew safe and also be without for the rest of the mission).
Result: Everyone makes it back to the ship alive.
Second Fireteam leader: Miranda (being a Cerberus leader her description also includes keeping the team alive).
Biotic specialist: Jack (being a primal biotic she has the power to do it well).
Squad: Mordin and Jacob (I wanted to make use of them this time because they didn't do much my first playthrough).
Result: Jack successfully keeps everyone alive and has enough power at the end to blast off the swarm while we all escaped. Mordin's incinerate and Jacob's throw was very useful against those husks! Miranda took a shot to the stomach coming through the door but shrugged it off and said she was fine. Everyone on her team made it.
Reaper battle squad: Thane and Samara (wanted to see them in action this time and also wanted to leave the heavy combat skilled holding the line i.e. Grunt to increase everyone's survival odds).
Holding the line: everyone else.
Result: everyone survives. I was amused this time when Shepard calls back to tell the other squad members to get back to the ship and Jack is the one who answers and says they are all OK. Thane and Samara didn't say any dialogue at all when I told the Illusive man to shove it but they survive the platform crash OK. Samara is the one who pulls me back on to the ship after I jump. Everyone survives.
So, I was even more impressed that time when I changed nearly everyone's roles and they all still survived. Mind you, I did have more knowledge at that point from here about who else can fill those roles well. But that was common sense anyway. I tried to prove that you don't need to send Mordin away with the crew for him to live as most people seem to believe.
I like to think that also your dialogue choices during the rest of the game had an impact on their survival - e.g. if you used paragon options like "We'll all make it!" during your hold the line speech or on the ship when they question their chances of living. Maybe even getting Mess Sergeant Gardener that good food raises crew morale and stuff... you never know... because even that guy that usually sits in the mess hall was OK on a medical bed at the end - wasn't he the guy that says he'll help Joker get through the crew deck and then you see a collector trample him or something?

Tech specialist: Tali (due to her tech expertise and I figured her suit would protect her in hot vents).
Two squad with me running to the door and opening vents: Legion and Grunt.
Result: Everyone survived. You can see while the door is closing and Tali trying to hack it, Garrus ran infront of the door and held off the bad guys.
Escort Normandy crew: Zaeed. (He just seemed badass enough to keep the crew safe and also be without for the rest of the mission).
Result: Everyone makes it back to the ship alive.
Second Fireteam leader: Miranda (being a Cerberus leader her description also includes keeping the team alive).
Biotic specialist: Jack (being a primal biotic she has the power to do it well).
Squad: Mordin and Jacob (I wanted to make use of them this time because they didn't do much my first playthrough).
Result: Jack successfully keeps everyone alive and has enough power at the end to blast off the swarm while we all escaped. Mordin's incinerate and Jacob's throw was very useful against those husks! Miranda took a shot to the stomach coming through the door but shrugged it off and said she was fine. Everyone on her team made it.
Reaper battle squad: Thane and Samara (wanted to see them in action this time and also wanted to leave the heavy combat skilled holding the line i.e. Grunt to increase everyone's survival odds).
Holding the line: everyone else.
Result: everyone survives. I was amused this time when Shepard calls back to tell the other squad members to get back to the ship and Jack is the one who answers and says they are all OK. Thane and Samara didn't say any dialogue at all when I told the Illusive man to shove it but they survive the platform crash OK. Samara is the one who pulls me back on to the ship after I jump. Everyone survives.
So, I was even more impressed that time when I changed nearly everyone's roles and they all still survived. Mind you, I did have more knowledge at that point from here about who else can fill those roles well. But that was common sense anyway. I tried to prove that you don't need to send Mordin away with the crew for him to live as most people seem to believe.
I like to think that also your dialogue choices during the rest of the game had an impact on their survival - e.g. if you used paragon options like "We'll all make it!" during your hold the line speech or on the ship when they question their chances of living. Maybe even getting Mess Sergeant Gardener that good food raises crew morale and stuff... you never know... because even that guy that usually sits in the mess hall was OK on a medical bed at the end - wasn't he the guy that says he'll help Joker get through the crew deck and then you see a collector trample him or something?
