I finished the game for the first time yesterday (late comer 
, girlfriend got me into the game, wasnt impressed by my love of Jack but i digress)
All members loyal, all upgrades,paragon(tiny renegade, I keep the base).
Every one lived.
Legion-tech, so hacking doors
Garrus-Leader, on a personl RP note, it was his redemption in my mind for getting his old team killed.
Jack-Biotic (also LI )

I saved everyone on my paragon playthrough.
On my renegade playthrough, kasumi died in the engine core room, thane got a pillar in his chest, jack got shot through the ship floor, tali died closing the door, zaeed was the leader, mordin died taking the other survivors back and miranda died on the way out.

I like to keep all my faithful friends alive, espeically Tali, my LI 
, so no one died! 
The first time was amazing, though. The chance that not just you, but your friends and close ones could die as well. To be honest, it was the bst ending to a video game I'd ever played! It was scary though, because Jack was the only squadmate not loyal, so I was worried she'd die. Here's what i did, with my squs being Garrus and Tali most of the time:
*Got all ship upgrades, so we all got through the drive unscathed.*
Garrus led the second team.
Legion was my hacking expert.
Samara was my biotic barrier (Because I couldn't risk Jack.)
Mordin led the survivors to the ship, because he's useless anywhere else and almost always dies as a vanguard.
Thane and Tali helped me bring down the final boss.
Mission complete!

On my first playthrough, I didn't lose anyone. Got all my crew back. My entire team survived. Jack wasn't loyal and I didn't have Zaeed. I picked Miranda and Thane for the final battle.
