The best thing to do is send Mordin back with the crew.

I figured it could be the crew isn't in the best shape (which is why they need to go back) Mordin is a doctor not really a soldier and thats why he seems to randomly die.

Actually, it is possible.  You just have to be trying to do it.  I managed to get everyone killed with all 11 recruited and 3 loyalty quests done.  You just have to be sure to stick the loyal people in positions that will get them killed, not do any ship upgrades, and never pick someone who is right for the job.

I would just like to add that Shepard will still die even if there is one party member left at the end. There must be at least two. I know this because I tried to get an ending where only Garrus and I survive. But even though I watched him jump onto the ship, he never appeared next to joker to help me up and I died. Also, I would like to try and figure out how the game chooses which squad member dies due to weak biotic barrier. The game seems to favor not killing Miranda whenever possible from what I've seen. Even in a role where its guaranteed that she should die if she isn't loyal.

Does anything happen (cutscene or otherwise) happen when a team member dies? I just finished my first playthrough and everybody lived, but I was wondering if anything significant happened visually if one of them died.

Some have cutscenes, others can just die, for me Tali died and I had no clue until the end.

I got the "No Man Left Behind" achievement on my first play through.

I got the armor, and shields, along with the cannons because thats common sense.
I got all loyalties because I wanted to do everything before hitting the relay...
Completed "Paramour" with Tali because she is sweet.
Picked "Legion" as Tech, because he/it is a fricken Geth, duh.
Picked Garrus for team lead since I remember him talking about leading an elite team to take down cime lords or something...
Picked Jack for biotic because she is beast mode.

For the team to stay behind, I left Grunt and all the strong characters just to be safe.

NOTE: I *was* playing on Veteran tho.
NOTE2: The fully upgraded Sniper Rifle tears down Harbinger Drones very quickly.

'No one left behind' achievement on normal.

All loyalty quests done before crew abduction. Jack and Zaeed not loyal (but Jack still had warp ammo), everyone else loyal. Ship armour, shields and weapon upgrades done (not medbay). Other upgrades not maxxed, but probably close.

Straight to Omega relay after crew abduction.

Phase 1
Vents: Legion.
Fireteam leader: Miranda
Squad: Tali and Samara

Phase 2
Crew escort: Mordin
Biotic shield: Samara
Fireteam leader: Garrus
Squad: Jack and Zaeed

Final phase
Squad: Tali and Garrus.

Full crew rescued and whole team survived.

Initial playthrough
Armor and Guns upgraded, shields not and didn't recruit tali or samara and sent Legion to Cerberus), all team members loyal
Thane died due to no shield upgrade (I had Jack in my Cargo hold team, so not sure if she would have bit the bullet instead had she not).
Vents: Jacob - died obviously
1st Leader: Garrus
My Team: Grunt and Jack - not that it mattered as I cloak ran through most of this to try hitting the valves asap to see if this could save Jacob
Crew Guard: Mordin
Biotic: Jack
2nd Leader: Miranda
My Team: Grunt and Garrus
Final Battle: Grunt and Garrus
Left at Gate: Miranda, Jack, Zaeed
Final Battle and Left at gate teams survived, though Jack seemed super pissed at me when checked in with them just before weak point fight lol... leaving me with Miranda, Jacob, Jack, Garrus, Grunt and Zaeed.

This isn't how I wanted it exactly, only reason Grunt was loyal was because I messed up the unlocking of the collector mission and then didn't realise that after I had the IFF that I had 2 N7 missions I could use to unlock the joker event.

When I say i messed up the unlocking of the collector mission I didn't know that you had to have 7 standard (non DLC) party members in active duty before it unlocks. Least that is what I've worked out anyway.

One of my upcoming playthroughs I refer to as 'Anyone that isn't human gets left behind... well... except Jack'

Only Armor upgraded, Legion sent to Cerberus and only loyal members being Jacob, Miranda, Zaeed and Kasumi (not completing this run till she available)
Jack and Jacob used during Cargo hold should see possibly Tali and Thane die during ship journey
Garrus sent down the vents and killed at the door
Samara sent back with the crew dying whilst saving them.
Grunt or Jack to get swarmed whilst Miranda is used as Biotic
Mordin to hold the line and die doing so as 2nd fire team leader
Whichever of Grunt or Jack doesn't get swarmed to die when teamed up with Shep and Miranda against final boss, the other loyal members defending the gates should survive.
Leaving me with Miranda, Jacob, Kasumi and Zaeed, oh yeah and a collector base still intact.
Cerberus Totalitarian Unit FTW

Here something for someone to try
All Ship + Weapon upgrades done.
Loyal Members Jack, Miranda, Tali, <insert one character here>
Everyone else not loyal
Vents - Tali
Your Team - 2 non loyal members
Fire Team Leader - Miranda
Crew Escort - the person you chose for the <insert one character here> spot
Biotic - Jack
Diversion Leader - Miranda
Your Team - 2 non loyal members
Final Battle - Tali and Jack

I reckon that should see everyone come home alive, though could possibly lose some at the gates.

Is there a way to choose which of the 2 squadmates gets taken by the seeker swarms or is it just random?

To have all the characters survive, you have to play some missions with them too. You can't ignore each character.

I did two run throughs. One where I didn't play with most of the team. The people I didnt use died. Then I reloaded and did some missions with them all, and we made it.v

Obviously you bring the person along when you do there loyalty mission... so not sure what the hell your on about unless you didnt do them.

At which point, its gaining there loyalty thats a factor, not whether you bring them to random missions.

Only 1 gets taken by the seeker swarm.  But yes, you do in fact choose.  The person who gets taken is always your second selected squadmate for the biotic barrier part.

Trying for an end where only Tali, Garrus, Zaeed, Mordin, and Samara make it. If I don't get any of the upgrades except for shield, I think I can get Jack to die instantly, right? Sending legion down the hole will result in his death, and maybe sending mordin back with survivors will keep him alive.

Keeping the biotic unloyal and picking miranda for the second person should kill her, according to Tydurden. Any ideas on how to kill off jacob, thane, and grunt while keeping the rest alive?

Finally finished tonight. Took me 35 hours on normal but I had all upgrades and full loyalty.

Tech: Legion

2nd Team: Garrus

Escort: Grunt

Biotic Barrier: Samara

Diversion Team: Garrus

Final Assault: I took Garrus and Thane with me.

After all was said and done, everyone survived except Mordin. The final scene showed him dead and getting trampled on. I'm gonna try again tomorrow with him as the escort so he won't be around to die. If that doesn't work I'll bring him with me for the final assault but with my luck he will be crushed by debris.

Ok, in one play through I gave Legion to Cerberus, and had not upgraded the ship at all, and only did Miranda's and Samara's loyalty quests.  Also I never did activate Grunt. AND (almost forgot) I never recruited Tali

Mirandas cause I wanted to see her get her kit off. and Samara'sso I get Morinth.

Anyway Durring the Omega 4 run, Garrus Died when the engine overloaded not Thane or Tali or Legion.

So Garrus is an option for death durring engine core overload.... but you do have to do **** REALLY wrong for that to happen.

I've played through the game twice now with a full Paragon and full Renegade Shepard (Paragon was imported ME1 Infiltrator on Veteran, Renegade was imported ME1 soldier on Hardcore)

I completed both possible endings to Mordin's storyline, stopping him from shooting his protege and keeping the data on Paragon, and letting him kill the poor sucker and destroy the data on Renegade. My first playthrough, I didn't have enough Paragon points to ensure Zaeed's loyalty, so he died attempting to escort Dr. Chakwas (who was the only remaining crew member) back to the ship.

Other than the Zaeed escort, my two playthroughs were quite similar.

Paragon:

Legion - specialist

Garrus - Fire team leader, 1 & 2

Zaeed - Crew Escort (KIA)

Jack - Biotic Specialist

Final battle - Miranda and Garrus

Renegage

Legion - Specialist

Garrus - Fire Team 1 & 2 Leader

Jacob - Crew Escort (survived)

Samara - Biotic Specialist

Final Battle Team - Grunt and Zaeed

Both playthroughs, Mordin was taking a dirt nap in the final cutscene. Loyalty and upgrades in my two playthroughs were identical, and my soldier's HMG combined with the Cain ripped the Reaper to shreds far faster than it died against my infiltrator. Also, my infiltrator had Mordin with him almost the entire game, so it seems unlikely that time played with Mordin, time spent against boss, or loyalty/upgrade status decides his fate. Will be reloading the Renegade playthrough and trying to get him through as the escort, will post my findings here if he is replaced by another randomly dead person.

During my first playthrough only one died.

I had all ship upgrades and all loyal beside jack, due to the catfight with miranda and not having an high enough hero score.

Only legion died which was in my team during the biotic escort part because i tought jack would've been the best choice due to his background, but what happened was she eventually stopped her shield at the very end and legion went kamikaze to cover us and got destroyed by the swarm.

I selected tali as tech and she survived with miranda leader of the second squad and noone died.

Then i picked garrus as leader for the diversive and used jack as biotic.

Depends if you wanting to do this RP style or not, would make sense for you to do any upgrades your loyal members can give you, but if you not bothered about RP, don't bother with the ship upgrades.

As far as I am aware the standard trio that die without the 3 main ship upgrades are Jack (armor not upgrade), Legion (shields not upgraded) and Thane (guns not upgraded). I would advise taking Tali with you to the Cargo Hold just to be sure though as I've seen her die during the shields part and that happens mid cargo hold fight.

Then you can send Jacob into the vents, he dead
Grunt as 2nd fire team leader, he dead
Non-Loyal Miranda as crew escort, she should die but the crew should make it all ok
Final Battle party whomever you want and they all should live.

@Skyzzuz Jack would have been ok if she had been loyal, due to her lack of loyalty she doesn't hold the barrier up long enough. If she is loyal she turns around and fires a biotic blast to clear the way of collectors coming behind you (as seen in the launch trailer)

I just don't get why Bioware hates Mordin so much? All upgrades and loyalty but Assault Rifles (4/5). Went to Relay immediately after attack. Everyone survived, after final boss, Mordin died. Seriously Bioware WHY! ;) *nerd rage*

I think I'm done re-playing until some can figure out why Bioware hates Salarians so much. :P

*edit*

OK followed the few posts above, and re-loaded my last auto-save. Have Mordin and Miranda at final boss battle, ALL survived. YAY!

Here's most of my play through as paragon if anyone wants to see, it's a huge file though @ 846MB:

Sorry it looks like I was wrong in my earlier post about my squad selection, I took Samara and Jack w/ me for the first part, not Miranda and Jack.

Here's my take on why Mordin seems to "randomly" die.

Think about BioWare's roots - in some of their previous RPG games such as Baldur's Gate and KotOR, there was a D&D element to the game, and it could be visible to the player. Basically, odds-based-outcome.
I'm betting in ME2, some of this is going on, but behind the scenes - the player doesn't see it.

For example, let's assume they used something like weighted averages to calculate effective character and/or squad strengths. Loyality has a massive weighting (say 50%). Each assigned power and what level could be weighted factors. Weapon loadout might be factors. Game decisions may be factors (but perhaps small ones).

Now, if the game throws all these weighted averages into a mix, it could possibly calculate a SQUAD STRENGTH. Then, during say the hold-the-door battle, that squad needs to be above a certain level to survive (perhaps here with a little bit of randomness, but not much). As an example, say the game calculates a squad strength of 80%, and the battle itself generates an DAMAGE value applied to that squad. If the damage strength is greater than the squad strength, then maybe the game further looks at the weakest character in the squad and either wipes him out or calculates a chance-to-survive based on that character's strength (again uses secret formulas and weighted averages).

If ME2 was using an internal game mechanic like this, then this might explain the seeming "random" death of mordin. It may not be just pure luck but based on internal game number crunching - if you put Morin on a squad with a low chance of survival (based on the numbers) he'll probably die because he's the least battle-hardend character. I betcha he survives if he's the escort because that mission is low risk and he's so hepped up on coffee it's go-go-go for him.
