Also, was wondering how Shepard dies?

If I remember correctly, no loyal squad mates results in Shepard booking it to the shuttle on his/her own. When Shep jumps for the shuttle, Joker catches him/her but can't pull him/her onto the Shuttle. So...Shepard lets go and dies.

I don't know if my contribution will further the cause, but it's another bit of data regardless. I brought them all home. My squad and crew combined and didn't lose a soul. I had full loyalty from all squad mates. I carried over a renegade Shep with Liara as LI from ME so I didn't pursue any romance option. I barely talked with any of my squad throughout the game. I only spoke with them when necessary and was fairly rude even when they were asking me to do their personal missions. Kind of a just cut to the chase brusqueness. I asked them for the upgrades and left them alone otherwise. Got every upgrade for ship, squad, armor and weapons except for the extra ammo for shotgun. I scanned just enough planets to get the resources I needed. Completed about 6 N7 missions. I paused just long enough to farm some platinum for Legions sniper rifle before I hit the relay. For the end mission my assignment breakdown was:

Vents - Legion. I was willing to lose him rather then Tali. If anyone was going to die I'd prefer to lose the new guys and keep my ME buds Garrus and Tali out of harms way if I could. *shrug*

2nd team lead - Miranda both times.
I recalled that when Jack came aboard Miranda told her that she was second in command to my Shep. I figured that the devs made a definite attempt to point that out on purpose. I guess it was in the nature of a clue-by-four straight across the chops and based my decision on that.

Escort - Mordin. He always seemed kind of scrawny to me and I figured it was best to just get him out of dodge before he hurt himself.

Barrier - Samara. I didn't trust Jack at all and for some reason knew I could rely on a Justicar to get the job done.

Personal squad - Zaeed and Grunt every time. These were my two heavies through most of the game.

I chose mostly renegade so I told Jack to kill the guy at the old base. I let Mordin kill his old buddy. I helped Garrus snipe the Turian. I did paragon Thane because I couldn't see letting his son go down that path. Like I said I didn't romance anyone and told the yeoman to keep it professional with no flirting. I played it as a no nonsense, pragmatic and hardboiled SOB. Great game and lots of fun. Now I'll import my paragon Vanguard w/Ash for my LI and give it another go.

What i have found ( using the same
characters over ) is that ppl dies when you take a lot of time to do
what you have to do, for example, if you take too much time on the
final boss one or more characters will die, the time you have depends
if you made the right choices, if you pick a tech expert wich is not
much of a tech expert you will have less time to complete the section
in order to save him.

No one will die if you spend too much time on the final boss (I should know), but you might be right about having less time to do things with certain people. I think I managed to save an unloyal Miranda (as the 2nd leader) after hearing her distressed radio calls and picking up the pace. But that doesn't exactly explain why Mordin can randomly die at the end.

I still think that Mordin's life at the end partcially depends on who is left by the door with him, or in other words those that Shepard doesn't take with him to fight the boss.

Well, maybe its the time for the whole operation, as i said i picked the same chars more than once and if i do things fast, no one dies, if i take my time moradin dies.

Great information here, and explains why I lost Legion my first playthrough (used Samara as the first fireteam leader).

Wow Mordin seems completely random. That's really strange that they would make him so different from the all the other characters, who are easy to keep alive and stay alive for obvious reasons.

I would like to add something up to the OP : this is regarding the ship upgrades and deaths. On my last play through where i was aiming for as many deaths as i could, i managed to hit the omega relay without having legion AND tali in squad. I went for IFF ASAP and the crew got kidnapped before i recruited tali. I sent the legion to cerberus. So in this case Thane dies at the debris field due to the lack of barriers and then Garrus dies when fighting collector ship. I had Zaeed though, so i think 9 is the minimal number you can go with. death count will be 3 in the movies, 1 in the first part, 2 in the second part. 1 guy to bring back the crew (or he will stay at the doors otherwise) and 2 in the team for terminator. Even though i had only one person guarding the door (loyal samara) she survived. And i think only way to Sheppard die is to have both of your squad mates die in the final battle, because they are the ones that will pull you up on the ship. Hope at least something i wrote havent been mentioned here yet :D

Enjoy and all hail the Blasto.

PS : For the pony

Jack was full loyal with a buddy of mine running biotic on full renegade. Legion was in his party and was the team member that died because Jack couldn't maintain the field.

Mordin does seem to be random, and it's sad that I couldn't save him.

My two cents on this, just finished my first playthrough and upgraded all of the Normandy's systems except for what Legion could offer, but if that's overridden by Garrus' Thanix cannon then its all cool.

I brought everyone home I'm pretty sure, I made sure I did almost everything I could before I went to get the Reaper IFF, so loyalties were all up (except for Grunt who was loyal but didn't have loyalty power unlocked.. I'll have to see if this is a bug reported elsewhere)

WIth regards to the team organization, I'd agree with most posts here;

Tech : Legion

Fire Team : Garrus on both occasions (Who also scared me in the first scene where he got shot in the gut but recovered well enough, the tough cookie)

Biotic : Samara

Escort : Zaeed ( I'd assume this really depends on whether or not you chose to save the crew, and how this decision can affect the survival of the team probably merits some investigation)

I also went the paragon path for both Jack/Miranda and Tali/Legion conflicts.

The team I took to the final baddie was Mordin and Garrus.

There cannot be an element of randomness to who dies or not because this can obviously affect ME 3.

It may or may not be an issue, but I finally had all armour/shield upgrades just before heading off. The only upgrades I didn't have were 2 levels of skin weave, and varying levels of shotgun/sniper/handgun damage. I also didn't have the 2nd level of the geth shield upgrade (damn you resources I was short 2 platinum!!1!)

I read on the Mass Effect Wiki that if you select "We should hurry" in the speech, then who survives holding the line becomes dependent on how long it takes to beat the reaper. Otherwise, it doesn't. Don't know if this is true or not, but I thought I'd bring it up as I didn't see it mentioned in this topic.

If it helps, here's my story:

Everyone loyal, all upgrades, nobody died on the ship.

Sent LEGION through the vent, GARRUS led the team, I took MIRANDA and SAMARA with me.
Everyone survived.

Sent ZAEED as escort, GARRUS led the team, SAMARA provided the field, MIRANDA and THANE came with me. Everyone survived.

Gave the paragon speech, was disappointed I didn't get to say "HOLD THE LINE", took MIRANDA and SAMARA with me. Blew everything up, Miranda quit Cerberus, crapped myself when I saw the bodies under the rubble, but everyone survived in the end. No casualties.

Edited to add: If Mordin is random... I didn't send him with the crew, and I left him to hold the line.... sounds like I had a lucky escape. I don't know what I would have done if Mordin had died... he was easily the most fun guy on the crew. It would surely have tested my resolve not to reload.

Theory: could Mordin's death be related to sending a squadmember to escort? On my all survived playthrough I didn't upgrade his omni tool but also didn't send a escort for Chakwas and everyone made it out alive.

My only addition was that in final boss fight my party was Grunt and Legion, both were loyal and had upgraded individual weapons. They survived (as did everyone else, but I happened to follow choices already posted).

Are loyalty and upgraded individual weapons required for final party members to survive?

My playthrough: All were loyal except Jack, and Jack was only non-loyal because of the Miranda/Jack scene.

Tali went in Vents, Zaeed led second team, Grunt and Garrus were with me.

Tali died.

Second section, Samara did biotics, Thane was "Distraction" and Grunt and Garrus was with me.

Thane died.

Jacob escorted crew back to ship, was fine.

Final boss, Samara and Garrus were with me, both survived, and no one else died.

Had all ship upgrades, and all individual party member upgrades EXCEPT Legion, who lived anyway.

For people with the Mordin issues, here's a question... Did you have him keep a copy of the genophage cure?

I did, and he survived. I wonder if not having him have that reduces his chance of survival.

If a team mate that is not loyal escorts the crew back to the Normandy the squaddie will die but the crew will live.

Here's my experience:

Started with fully upgraded Normandy, but delayed long enough for the entire crew to be liquefied.

Sniper rifles - 5/5 +2
Heavy pistols - 5/5 +2
SMGs - 4/5 +2
Shotguns and Assault Rifles - 3/5 +2
Biotics 3/5 +2
All omni-tool upgrades reseached.
All shield upgrades researched.
All personal upgrades (Mordin Omni-tool, Jack implants, Legion rifle and Grunt shotgun) researched.
Everyone is loyal - used Charm to disarm confrontations
Zaeed is recruited (and loyal).

First phase:
Tech specialist - Tali
1st fire team leader - Garrus
My squad - Mordin and Jacob
no casualties

Second phase:
Biotic specialist - Jack
2nd fire team leader - Garrus
Escort - Samara
My squad - Mordin and Jacob
no casualties, Garrus got hit but lived

Final phase:
Chose "We better hurry" option and gave full paragon speech.
My squad - Garrus and Jacob
Took moderate amount of time with the boss, two reinforcement platforms arrived in the first stage.
Chose to destroy station.

Result: Everybody lives, including Mordin.

As for Mordin's personal issues, I prevented him from murdering his student and told him to keep a copy of the cure data. Advanced conversations to the point where he pre-emptively rejects your romantic advances.

Okay, I actually didn't know that people could die under the falling debris :

Makes total sense that that's how Shep loses his "jump buddies" though.

I've read this whole thread and people with totally different inputs for omni-tool, personal quest, team choices, have had Mordin live. And people with similar choices have had Mordin die. So yeah it's either random, a bug, or some incredibly obscure trigger.

I dont know if this has been said by anyone, but sending a loyal Mordin with the crew saves him. I also left Loyal Legion to hold the door and didn't upgrade his rifle to the Widow yet he still survived.

Yeah that seems to be the one way to make sure. Wish theyd tell you though..."if you don't send Mordin back with crew he has a high chance of randomly dying".

Vents  :  Legion, I figured he was one of the best Techs and I didn't want to lose Tali.
Leader/Fireteam Leader :  1st - Garrus, 2nd - Miranda
Biotic :  Samara
Escort  :  Tali
Boss Squad Mates :  Grunt and Miranda

No upgraded personal weapons.

My ONLY casuality was Jack, because I  forgot to upgrade my armor on the ship. I  had the cannon and shield though,  I don't really miss Jack - She was interesting but I am not upset about losing her, considering I never used her in my squad or anything.

So without upgrading my squads individual weapons and letting Mordin hold the line I was able to have everybody make it out except Jack.  EVERYBODY would have survived if I just upgradded that armor....  Oh well, I'd have to say I was lucky.
