I was mostly thinking of this kind of playthrough becoming a strong roleplaying element in ME 3: Sheppard saves the galaxy (again) but is now haunted by the lives lost in doing so... Maybe build this on the deaths of Wrex, Ashley/Kaidan, and the council in ME 1 and now we have a very interesting role to play in. Maybe future potential crew members will think Sheppard is cursed, or maybe the Sheppard you play won't bring himself to put anyone else at risk...
I might be the only one that finds the idea worth pursuing, but maybe I'm the only one who would be so cruel to a character they've spent 40+ hours creating >: D

From what I understand, losing all your other team members will force Shepard to die as he tries to jump onto the Normandy. I haven't tested this completely myself, but I don't think it's possible to be the sole survivor.

Team - Garrus and Grunt (whole time)
I purposely mixed things up a bit to see what would happen. I had all ship upgrades and most weapon/armor upgrades (I didn't bother upgrading weapons I didn't use). All crew members except Zaeed and Jack where loyal to me. Legion died closing the doors (BOOM! HEADSHOT). Zaeed, although not loyal, managed to get everyone through safely. Thane died leading the second team and no one died in the swarms because Samara is OP. Tali and the crew made it back to the Normandy safely. After the boss fight, I saw Jack's body lying on the ground in one of the cut scenes so she died to. All in all I was only upset about Thane dying. i thought he was pretty cool lol.
My playthrough pretty much falls under what you've stated, Ken.

That's probably true from what I'm reading on this thread, but I just wanted to see what other people might think about it. Maybe I can get the same effect by saving one squad member but none of the crew. It's an idea that I'll probably use to pass the time til ME 3.

All crew members loyal, all individual member upgrades completed, all ship upgrades completed, almost all weapon/armor/prototypes researched. I stopped Mordin from shooting his old research team partner and DID give him a copy of the genophage data.
Sent TALI in the tubes, GARRUS leading the fire team and had SAMARA and ZAEED with me.
Used GRUNT to escort crew back to the Normandy.
Then had SAMARA hold off the swarms and had ZAEED and MIRANDA with me. GARRUS led the distraction team.
For the final fight I had SAMARA and ZAEED.
Everyone survived except Mordin, who was shown randomly dead in the group holding the line.
Really lame if it is a bug or a completely random thing, I really liked him as a character. I'm tempted to reload and send him as the escort, but in a way I already have the emotional reaction of watching him die so I might just deal with it and make his sacrifice mean something in ME3...

This doesn't seem right: I did my very last loyalty mission (Legion) right after the kidnap (we were already there) and went straight to the Omega 4 after, and ended up with Kelly, half the crew, and *one* of the engineers (the girl) dying.Ken Ueno wrote...

Suicide Mission Result after losing Thane on ship
Vents: Jacob (did my best to sprint to each valve and shut it off, not sure if this had an affect as probably a few collectors still alive when pressed the button)

Final Battle team: Grunt and Garrus
Surviving Members: Miranda, Garrus, Jack, Mordin, Grunt, Zaeed
Think I did ok, all things considered. Though when Jack said they were in a bloody mess I really did worry that I would lose at least one more during that final bit but nope, all those left to defend the door survived
Funniest thing was, due to me not realising I had one system with some planets unexplored I ended up doing Grunt's loyalty quest. Reason why he ended up in my final battle team was because he'd done a good job earlier on during the biotic stage, I decided to keep him for final battle... funny watching a femshep slide down slope chasing after grunt and managing to grab him and pull him back up from the abyss. What was even more funny, he returned the favour when going for the ship :happy:

Picking the right person for the right thing gives you more time or less time, i have tested some of this things, for example take any biotic to hold the barrier and take ALL the time you can killing the mobs and someone will die.

Had everyone survive on my run, played on Veteran difficulty and took what I thought was my dear sweet time fighting the Reaper. Most of the stage was destroyed before he finally went down. Weighing in on the Mordin thing, I disagreed on what he thought about the Genophage.

I went into the suicide mission with all Normandy Upgrades and all but Samara and Miranda as loyal teammates. I was able to save the entire crew, because I completed Legion's loyalty mission before they were attacked.
Legion as Tech Specialist
Garrus as Fire Team Leader
Thane and Miranda for Squad
Jack for Biotic Specialist
Garrus as 2nd Fire Team Leader
Grunt as Escort
Thane and Miranda for Squad
I believe I did a full Paragon speech before leaving and only lost Miranda (as un-loyal squadmate) and Samara (presumably because she simply wasn't loyal). I have to assume there are more subtle variables being decided in the characters loyalty missions and maybe in some of their dialogue on the Normandy or main missions.

Most generic gear upgrades complete (missing a point or two of skinweave, some weapon damage)
All Characters Loyal.
Full Paragon speech.
Tech Specialist: Tali
Fireteam Leader: Miranda
Team Members: Jaeed, Samara.
Result: Steamrollered the collectors, no deaths.
Biotic Shield: Samara
Fireteam Leader: Miranda
Team Members: Jaeed, Jack
Result: 1 Medi-gel used to wake up Jack after she charged Harbinger, no deaths.
Hostage Escort: Grunt.
Result: All survive.
Final Team: Jaeed, Samara.
Result: Used 1 medi-gl to wake up Jaeed after he got stuck on top of a piece of cover and focus-fired by an entire wave of collectors. Could have killed the endboss quicker. Told Illusive man to go stuff himself, blew up the station. Mordin apparently died 'holding the line'

I think that it's tied to how long you fight the human-reaper. On my first playthorugh I used the nuke gun and finished him off pretty quickly -> everyone lived
2nd time (restarted mission with mostly same parameters: only changes: Tali instead of Legion in vents, no escort for Chakwas, chose to give the station to Cerberus) it took longer since I used a different heavy weapon -> only Mordin died.
Maybe if you fight longer, the least protected squad mates will die one ofter another. I'm guessing Tali or Thane if you battle the human-reaper very slowly.

Space Battle

-No Thanix Cannon: Normandy charges in with main guns; Thane dies.
Base Assault
For the assault itself, there appear to be three types of characters that are required for the various key roles:
-If the Tech Specialist is not one of those listed below, they will die at the first rendevous.
-If the first Fireteam Leader is not a leadership character, the Tech Specialist will die at the first rendevous.
-If the second Fireteam Leader is not a leadership character, they will die at the second rendevous.
-If the Biotic Specialist is not a powerful biotic, one of your current squad members will be swarmed by seekers and die.
-If the crew survivors are not escorted, they will die (any character can do this and the crew will live).
-If the Heat Exchanger valves are not opened in time, the Tech Specialist will die and you're forced to restart.
The following characters are the only ones that will succeed at their respective roles:
Tech Specialist
* Tali
* Legion
Leadership
* Miranda
* Jacob
* Garrus
Biotic Specialist
* Samara / Morinth
* Jack
Crew Escort
* Any loyal character
Disloyalty
Not having a character's loyalty has different effects depending on what you do with them:
-Having disloyal characters in the above key positions will result in someone dying.
-A disloyal crew escort will sacrifice their lives to protect the crew (the crew will still make it).
-Disloyal characters holding the line while you fight the last boss may or may not die (still checking).
-Keeping disloyal characters in your active squad prevents them from dying, except for...
-In the final boss fight, disloyal characters in your active squad will be crushed by rubble after the fight and die.
-If all team members are dead, Shepard misses the final jump and lets himself fall to his death rather than risk Joker's life.
Kidnapped Normandy Crew
If you do not go through the Omega-4 relay quickly enough after the Collectors kidnap the Normany crew, the captured crew members will die off in the endgame. Watching Kelly get liquified was particularly horrifying. Who dies depends on how many missions you did after the Joker level:
-If you do one mission or you use the relay immediately, everybody survives
-If you do two missions, Kelly, engineers Donnelly and Daniels, and half the crew die
-If you do three missions or more, everyone except Dr. Chakwas dies
Legion's Loyalty Mission
Note that if you activate Legion and learn about his loyalty quest immediately after the Reaper IFF mission, the Collector attack / Joker level will wait for one extra mission before triggering. This allows you to complete all the loyalty quests before the crew is even kidnapped (assuming you did the other ones already).
The X-Factor
Some people also report having trouble keeping Mordin or Legion alive at the end where they must hold the line until Shepard kills the last boss. This may be related to the member-specific upgrades you can perform (Mordin's omni-tool upgrade, Legion's Widow rifle, etc), or how well you got to know them over the course of the game via conversation, or how long you took to complete the fight, or how many other team members are left at that point. Still looking into this one.
So if I follow this I will get all team members and crew members 100% alive and safe?

I think it is tied to how long it takes to kill the terminator because my adept took a bloody long time on insanity mode.
