One other thing I found. I didn't have Legion's loyalty because I sided with Tali, and I hadn't bothered purchasing his upgrade because I was too lazy to get the 15k platinum. The first time I went through, I decided not to save the crew, and all of my squad survived. I then decided i'd rather have the crew, so sent Grunt with them but did all my choices the same. This time, Legion did die at the final fight.
To summarise, if you're finding one of the characters dying on defence because you haven't purchased their upgrade, try not saving the crew and they might survive, letting you get your achievement.

All members loyal and all ship upgrades, went straight to omega 4 after the collectors, boarded my ship(scary with no weapons right?)
Sent legion threw the pipes, and made sure not to make him wait long
had garrus lead the squads both time as it said he has exsisting experiance leading gurellia assualts before, plus tbh he is a legend
my squad was miranda and grunt, i liked their personalty and they complemented my shepard well,
used these 2 also for the final fight. Glad she was loal could see my head getting sprayed for not doing as TIM wanted.
managed to get to the crew in time and save all of them
and used samara as biotic sheild as the asari are the best race for biotics.
and finally used jacob as the escort as i believed his biotics would help get them through.
also to add to the confusion to why legion may die when trying to close the doors may be down to the time that it takes to do so as i assume u did not use him in the vent. As a geth he has more processing power than EDI so he is alot quicker than say thane would be in getting the door closed, as it was malfunctioning it would of taken time to continue to try and bypass the door which is where he would be the better choice. which is why i beleive that the delay in the door costs his life. to mordin dying, havent a clue and i feel sorry for those of u that lost him, such a funny character ^^
on a final note, would like to thank Bioware for such an amazing gaming experiance and i'm waiting on mass effect 3

tech people (modrin tali and legion) might die in place of squad leader if you pick a bad leader.
escorts almost always live...

Not true. You can use paragon to bring him to your way of thinking at the end of his loyalty mission (as far as i know you need to have 100% though, it worked for me). As far as end-game, I had him escort the crew back and he made it just fine. I didn't really use him much during the main game, but his bonus power (Inferno Grenade) is just awesome, and my soldier shep took that in advanced training.

Zaeed escorted the crew back to the Normandy safe and sound. Zaeed is awesome.
Samara was my specialist, and I took Miranda and Jack in case Samara needed help. Jacob led the second team in case they suddenly needed the same biotic protection.
Final party was Miranda and Mordin.

I had jacob true the veints. (he died)
Thane leading both teams (he died)
Jack escorted the team back (she died ) But she was not loyal
Samara for the field, and for the end battle i took Miranda and Tali.
And after the crew vansished i did no extra missions. Just did resources... and upgrade. I think everything is fine so long you dont do extra quests.

Possible that it depends on who you are taking to the reaper?
I took Garrus and Amara, Modrin died while defending the gate - maybe one assault rifle was missing and they had not enough firepower?
The guys who get the achievement don't take two characters with assault guns in the final battle - maybe this is the decisive point?

I also had all upgrades.
But yeah im not asking for a guide on how to get it im asking if anyone else has seen this inconsistency ,where something that worked for others didn' work for them.
He does count for the achievement. I used Garrus for firt leader and then Zaeed. Zaeed died, but went out like a hardass. Everyone was loyal, but he was the only that died. And I didnt get it

Normandy fully upgraded, other than med-bay.
Majority of weapons/armor/tech/biotic upgrades completed.
Loyalty of all members, left Legion unconscious in the ship and went straight for the Collectors after the abduction of the crew.
I did my best to keep the people who conflicted with each other most away, i.e. Jack and Miranda.
Phase 1
-Tali in vents, Jacob leading second team. My team had Thane and Miranda.
Phase 2
-Jack as my biotic shield generator, Garrus leading second team. My team, Thane and Jacob.
Phase 3
Grunt escort crew back to ship, my team consisted of Miranda and Mordin. The reason I brought Mordin was he had died at the door on my two previous attempts even though he was loyal and upgraded, so I decided to risk bringing him along with me instead.
Got the achievement, which was the last one I needed. I have now officially beaten Mass Effect 2 three times in one week. Now, its back to real life as my week off work ends tomorrow =(

I used Tali, who was my LI, may have made a difference to open the door.
I used Garrus for the leader fist time because of seeming tension with Miranda as Leader, but the 2nd time I used Miranda.
Zaeed escorted the crew back.
Justicar was my biotic escort.
Final fight was Mordin, and Legion.

Legion is the best choice for the Vent. Tali is suseptable to the heat, so you have to move really fast and while Thane can withstand the heat longest, he lacks the tech ability to overide the door when it jams and is highly likely to be shot trying to close it manually.
My concern is that I also lose the crew. The first time I lost them all because I thought I had time to buff my research before going through the relay. The second time I only took the time to do Legion's loyalty message and apply the bonus talent and retrain researches. I still lost my hacker at the door, and I "only" lost half the crew.
Best advice I can offer is don't go after the IFF as soon as the Illusive man tells you about it. Hold off, build up your squad and upgrades then go get it.... and whatever you do, go straight through the relay when the crew is abducted, don't delay a second.
P.S. I assigned Samara as team leader, based on 400 years of Justicar experience. I see now that may not make sense as she is a lone samurai, not a leader - but will THAT make the difference in saving either the hacker or the crew?
Yes, the clue of who is best suited to what is in their backgrounds. Garrus wil spend much of his loyalty mission telling you about "his squad" *hint, hint.*

Picked grunt as the 2nd team leader and he died...garrus escorted the crew ok. Tali died during the final right.
This is the most...annoying yet interesting and intriguing part of the game...some hints would be awesome!

I had everyone loyal and I had almost every weapon upgraded along with tech and biotic ability.
Since I care for both Tali and Miranda I keep them with me the whole time to make sure they stayed alive plus they always say some cool stuff together.
Legion did the vents
Jacob lead both missions
Thane escorted
Samara was the biotic, Miranda is good but lets face it Samara is the Justicar after all.
It keep me on the edge of my seat but everything worked out in the end.
Although I dont want to see anyone die I am going to mess around a bit to see how people die just to say I have done everything possible in the game. Nothing like dummy saves to test stuff out when ME3 comes out.

Saving the entire crew appears to require going through the relay immediately after they are taken. I think that requires doing Legion's loyalty mission immediately after acquiring the IFF (before it is installed). I must have delayed long enough doing ship conversations that I did not have that option.
I've backed up to an earlier save and will fix that.
I took Mordin and Thane with me in both phases and they did spectacularly. The mix of weapon types and skills let me ignore them and concentrate on the main objectives while they cleaned up handily. (I should probably mention I am playing on Casual.)

Yes once you pick up the IFF and get Legion, Activate and talk to him right away, He will give you his loyalty mission almost immediately. After his loyalty mission you have to deal with Tali and Legion, Make sure you're full Paragon or Renegade, It also helps with Miranda and Jack. Then you'll hear that EDI and the Crew are installing the IFF. When you click on the Galaxy map, The Collectors will Attack.

- Have either Tali or Legion as your tech specialist in the vent (they have the best tech ability)
- Have either Garrus or Miranda as your fire squad leader (they have best leadership experience)
- Have either Samara or Subject Zero as biotic specialist (they have best biotic ability)
- Have either Grunt or Zaeed escort the crew back to the ship (they are independent hard nuts)
- You may need the loyalty of these people (not sure), but you don't require all of them
- The crew do not count toward the achievement, but if you wanna save them, enter the relay as soon as they are captured
(So, try to acquire and loyalize all the squad members you want, before getting the Reaper IFF, and if you want Legion's loyalty, there is usually time to get it, before the crew are kidnapped - if you have a save near the Reaper IFF, you can usually get away with acquiring the achievement pretty fast).

2). - The choices made during the loyalty missions (do you paragon interrupt characters from doing bad stuff?)
3). - The amount of time you spend talking to loyal characters to learn about them (unconfirmed, people have got it without)
4). - Whether the fire squard is powerful enough to protect itself, and the doors while they are closing (unsure)
5). - How long the person in the vents has to put up with the heat before you open a valve (unsure)

Tubes: Legion
1st fire team: Miranda
Biotic sphere: Samara
Escort: Mordin
2nd fire team: Miranda
End fight: Legion & Thane
I picked Thane to the final fight squad to prevent him from dying in the ground team but now I'm not sure will he die anyway if I try another time with Legion & Garrus... Would suck really much to start over from scratch because of this achievement.
