Tubes: Legion (Thane + Garrus)
1st fire team: Miranda
Biotic sphere: Samara (Legion + Garrus)
2nd fire team: Miranda
Escort & final battle: Mordin (Legion + Garrus)
Snipers really kicked butt, I must say. Especially Legion's geth sniper rifle is insane.
EDIT: I would like to underline that it seems to be possible to get this achievement even WITHOUT everyone loyal. All ship upgrades done, all armor and prototypes researched, most weapon upgrades researched; missing only sniper 5/5 and SMG clip size I think. Still, snipers ruled over everything despite being only 4/5.

1st pass (did this three time in a row actually just to see if it was a random thing):
Vents: Tali
2nd Fire Team Leader: Garrus
My Squad: Grunt and Miranda
Tali got shot in the face
2nd pass (did this combo twice, again to see if it was just random):

2nd Team Leader: Zaeed
My Squad: Grunt and Miranda
Legion got shot in the face
3rd pass:

2nd Fire Team Leader: Garrus
My Squad: Grunt and Miranda
Changed my response to Miranda's comment about everyone knowing this was probably a one way trip after Joker crashed us into the base. Said "I'm not throwing my life away" rather than chosing the top right response.
Tali lived.
In the end I used the following teams, answered "I'm not throwing my life away" in the afforementioned dialog and "Hurry Up" rather than "Hold the Line" after the biotic barrier section, and every one lived:
Vents: Tail
2nd Fire Team Leader (both times): Garrus
Escort for Crew: Mordin
Biotic Barrier: Samara
My Squad for entire mission: Grunt and Miranda

Get armor ship upgrade from jacob
Get Shield ship upgrade from tali
Get Weapons ship Upgrade from Garrus
Get All SQuadmates loyal this requires a high paragon or renegade
WHen squad squabbles use your high renegade or paragon option to end it
If you want your entire crew to survive when they get abducted you have time for legions Loyalty mission then off to Omega relay
During Final mission Send Legion to infiltrate/First team leader Garrus or Miranda
Next Distraction Garrus/Escort Mordin/Team leader Miranda/Biotic Jack or samara/morinth
Final choice leave miranda and garrus to cover the door with the rest of the squad
Take whoever you want win then no one left behind
THIS IS NOT THE ONLY WAY TO DO IT

For the vent, you have to have a tech expert (meaning someone who can hack, ie Legion or Tali)
For the fire teams, you have to have someone who's experienced in leading other people (Jacob, Miranda, or Garrus have led combat units in the past, the rest of the squad were loners, followers, and non combatants) Choosing someone unsuited to leading the fire team will get your tech person killed regardless, and will get them killed themselves for the second round.
For the Barrier, you have to have a specialized... POWERFUL... biotic, not someone who just uses biotics in addition to other skills, that means Samara/Morinth, or Jack, anyone else doesn't have the energy pool to give that one final push.
For the final fight, leading the crew out, and for each of the above choices, you have to take someone who is loyal to you, otherwise, they won't go the extra mile to help you succeed.
With the distraction team, it seems as though you have to leave at least one tough fighter for them to live, Tali and Mordin won't make it on their own, Thane, Garrus, and Jack being left with the distraction team will cause it to survive. Not sure on the other team members, as I've only done one play through that wasn't a 100% survival, and that one was only to see the suicide ending.
I've been able to finish with 100% survival with 4 loyal team members, Tali (my vent runner), Garrus (Fire team leader both times) Jack (Barrier) and Thane. I took Garrus and Thane with me on the final boss, sent Tali back to the ship with the crew, left Jack with the distraction team, and everyone survived.

This was my set up for the final mission:
Legion as specialist
Miranda as first fireteam leader
Garrus as fireteam leader for part 2
Grunt as escort
I used all the valves in time.
Me, Garrus and Samara were in the final party.
Jacob died for no reason in the end. This confused me.

My Shep was a Soldier with the Revenant LMG. All my companions were loyal and I went to save the crew ASAP.
Here's my team set-ups and the reasons behind them.
Vents: Legion
--Geth super hacker who doesn't need atmosphere or gravity... and who knows what could happen in the vents
2nd Fire Team Leader: Miranda
--Her overload and warp would be good to keep others alive as when I had her in my team
My Team: Garrus & Tali
--Facing down a threat with my old friends like old times. Garrus' Concussion shot and Tali's Combat Drone... very helpful.
Escort for Crew: Samara
--A Justicar would help those in need
Biotic Barrier: Jack
--The "most powerful human biotic in the galaxy" seemed qualified for the task
My team: Garrus & Tali
--Again with my old friends. To Hell and back.
2nd Fire Team Leader: Miranda
--Again with her skills and powers
Finale squad: Garrus & Miranda
--I was tempted to keep Tali but I figured a "hold the line" scenario could use helpful distraction combat drones that go for the optics. Miranda's Warp/Overload combo was great for dealing with Collectors in the end.
I'd been spoiled from tech forums and from a friend only in that it's possible for people to die no matter their loyalty so I was sweating bullets during every cut scene with my squads. In the end, everyone lived (including crew). I got the ending I wanted for my carry-over save for ME3 on my first try. :-D Now it's time to wait a couple years till the end of the Trilogy, heh.

Legion through tunnels, Garrus as back up squad leader. Got to the vents asap and noone died.
Jack did the barrier, Miranda did the back up squad and Samara escorted the crew back.
Everyone stayed to watch the door while Tali and Miranda went with Shepard for final boss. Tali almost fell off the platform at the end but Shepard saved her :-)
Overall it seemed fairly logical. You even get vocal cues.. ie pick Garrus for fireteam and Miranda says "at least he knows what he's doing". Pick Legion for tunnels and Shepard says "you can do this, you can hack through anything". etc.

Pretty sure the conditions are:
Loyal Tali/Legion for the tubes (anyone else, even if loyal, dies)
Loyal Miranda/Garrus/Jacob for the fireteams (anyone else, even if loyal, dies)
Loyal Jack/Samara for Biotic (anyone else, even if loyal, dies)
Any loyal character (anyone have someone loyal die here?) for the Escort.
The only real issue seems to be the door. What determines who lives/dies there? Maybe to do with if you've had anyone die before that point (crew included?).

Everyone was loyal. All upgrades were done except for 1 shotgun and 1 smg damage upgrade.

Vents: Legion. I figured if Legion admired me enough to wear my armor, he would go the extra mile. I opened all the valves with plenty of time.
My Team: I brought Miranda and Thane with me so I could use her Overload and he could help me with fast kills.
Fire Team Leader: My boy Garrus. I taught him everything he knows, I knew he wouldn't screw it up. Basically, I assumed anyone who had tried to get into my femShep's pants was loyal enough to survive.
Escort: Thane, again, on the basis of my pants I figured he wanted to live long enough to try again.
Biotic Barrier: Samara. You'd be amazed at how well the pants test worked to determine loyalty.
Team 2: Miranda and Tali - Overload and Combat Drones to cover me while I did stupid things like standing up to snipe.
Fire Team 2: Why mess with perfection? Garrus again. He didn't fail me.
Finale Team: Miranda and Garrus. I needed backup I could trust and that could do damage, and frankly, Miranda's Overload was addictive. Garrus and I work really well together. Too bad he's still not getting any.
Saved the world. Everybody came back. Except that tart who snapped at my Kaidan. I didn't even imagine that ending for her, but hey it worked for me.
I did notice that the choice of Fire Team leader helps determine whether the specialist in the vents survives or not.
Also, the quickpost thing does weird stuff to my line spacing.

Not weird at all: once in, if the leader is appropriate calls for suppresive fire so that Legion/Tali can do his job, without a proper leader everybody rushes in and collectors swarm the entrance. Like Kal'Reegar said, hacking a door is quite difficult with somebody firing rockets at you. Deadly difficult.Chillwill wrote...
Why would that matter, the whole other team is with us behind the door, Legion dies while closing the door with us, after all parties had completed there part.
Really weird if that factors in.

That is correct up to the point about Grunt and Zaeed. You need to them to hold the line since they are considered the strongest defenders. Send Mordin back with the crew. Make sure to leave Grunt, Zaeed, Garrus and Jacob as the main defenders... the game considers them the toughest characters.
