It happens only rarely, but during her loyalty mission it happened almost every line. She'd talk the first half normal, then her voice would drop pitch and she'd sound extremely manly.
Miranda's voice sometimes drops in pitch and makes her sound like a man

that's usually due to the time dialiation in the game; at least that's my hypothosis. Whenever you "storm" (sprint) or at other random points in combat (I assume when you melee someone or when an explosion hits near you), the game distorts sound to make everything appear as if it is in slow motion.
Again, this is just my theory, as I always notice this whenever I storm.

Yeah, I notice it happen when I use Adrenaline, not Storm (yet, at least), but this is happening when I'm just waltzing around and she's commenting on things. It doesn't happen when I'm engaged in dialogue with her, though.
Just wanted to bring it up as a potential issue.

Hmm, maybe it's the location? some planets might have a really really really thick atmosphere that causes sound to travel slower.

I notice this issue in any voice clips that play within the first second or so of me going into my scope with a sniper rifle.
