Of my five characters, the only portrait that uploaded is the one where I didn't change the portrait settings -- angle, zoom, etc. at all.
Just chance, or does that hold true for anyone else?

No, that doesn't hold true.
I can see that, like me, you are on the Xbox 360. My first character's portrait uploaded without issue, but my most recent character's portrait has failed -- I adjusted portrait settings on both of them.
