-First of all, every crew member most be loyale to you

-secondly , make sur you made every possible upgrades on the normandy ( helps with the cinematique after passing the omega 4 relay ) so that nowone dies because of the collectores

-third, Name garrus your second squad leader and take tali as specialist

- fourth, after making the gameplay , name samara as the tank ( escort biotics shield ) and take  Miranda/Grunt  as  teamates ( mordin and thane a tendance to suicide ) 

-Fifth, take Zaeed as escort for the normandy crew

Thats about it, this is most likely your chance of sucses and if it dont work, it's not my fault
just try your combination

Grunt works for the escort as well. And the team leader for the distraction (the second extra team you have to assemble) can be jacob. The first specialist can be Legion instead of Tali, if you like.
