I already know that you can fail Tali's, Samara's, and Zaeed's, but what about the others?
Can I fail loyalty missions?

Er... not technically. No matter the outcome, they'll always be "loyal".

You can lose Tali's loyalty by handing over evidence that her father is guilty to the Admiralty Board, thus squashing their plans to launch an attack on the geth, keeping them around to find the reapers. So it really depends on your idea of 'fail'. Even though I had enough points to Intimidate the Admiralty Board and gained Tali's loyalty, I still chose to hand over the evidence, because that's what felt right for my character and my story. I really don't like playing the game as a 'game' and trying to work the system to get the outcome you want, I prefer the make the choices my Shepard would make, and ignore how the game works as a game, and living with the consequences, good or bad.
You can lose Zaeeds loyalty by refusing to go after his target and instead helping the hostages. This will result in his target getting away, leaving him frustrated and not loyal. Apparently you can persuade him into loyalty at some point, but I guess I didn't have enough points at the time and didn't see those options.
They're the only two I've failed to get the loyalty of, but there might be more. It seems to be impossible to lose Grunts loyalty on his mission, since you don't have any real choices that affect the outcome, same goes for Jack (I think).

You can lose Jack's loyalty if you side with Miranda when the two of them have a spat too...

You can also lose Thane's if you fail his mission.

You can get Zaeed's loyalty with a charm/intimidate check, but I hear your paragon/renegade levels need to be pretty high. I just did his near the end and at that point, I was at 100% paragon. Had no trouble getting his loyalty despite saving the workers.

You can fail Garrus' loyalty mission by tipping off his target (and not having a sufficient Paragon score to dissuade Garrus from his vengeance).
It is probably possible to fail others by making obviously "disloyal" choices. I haven't tried it on all of them.

Same here. I followed the Krogan into some building, watched him and then followed him into some nightclub i think and next thing i know i failed and saw a vid of Kolyat killing his target and getting away. Now Thane is permantly sulking.
