I've played through three-times now, here's what I've noticed:
1). Events are at least 'semi-random'. I only have evidence for this on concurrent playthroughs for me and my friend, both went full Renegade, both had full loyalty and upgrades, and even made the same decisions (Legion hacks, Garrus Leads, Garrus leads distraction, Jack for Barrier, Tali escorts). : I lost Mordin, he lost nobody.
With that said, here are the most common easily avoided pitfalls:
1). When selecting 'team leaders' for the distraction and the second bit, the keyword is 'leader'. Characters with high leadership skills and an understanding of a team are better here. Basically, this is a fancy way of saying 'Miranda' (who is a high-ranking Cerebrus member who's done a lot of leadership) or Garrus (the dialogue is a dead give-away. 'Lead through experience'. If you look at Garrus flavor text while selecting, it literally says lots of experience LEADING A TEAM).
2). Select 'pure' characters for the hacking part. Strangely, I've had Legion make it through unscathed and Tali, but I've never heard of Tali getting killed, so, all failing, try Tali.
3). You've met exactly two super-powerful Biotics in the game. Assuming they're loyal, pick Samara or Jack--I've never heard of Jack dying.
4). Choosing which character to send away doesn't seem to have a chance-on-death, but it keeps them safe. I lost Mordin twice, so I sent him home.
5). If all else fails and you can't seem to keep a particular party member alive: Just shove them in your party. They won't die as long as they're in.
3 and 5 aren't quite correct. My roommate lost Jack (used as biotic, was loyal) at the biotic part, and Grunt got carried away by swarms at that point. I'll double check with him to make sure I got that right, though. This is second hand at this point.
EDIT: Nope, I was wrong. Jack wasn't loyal, which explains it.
