A series of in ship quests that affect the moral of the Normandy's crew, either building it up or bringing it down.
An antagonist to your romantic interest, to spice things up.
Tests of friendships and loyalty that are based on character homeworld politics.
Reputation playing a bigger part in everythig that you do, if you are mostly Paragon some quests won't be available to you, the same applying if you are mostly Renegade.
Missions with NPC characters like in The Shadow Broker, a covert ops mission with Aria, an epic adventure with Wrex and so on with other important NPCs.
Friends outside the Normandy that provide intel that can be used on assignments and missions, their trust is earned of course through assingments during missions.
The Kobayashi Maru type mission that tests your leadership skills when faced with unavoidable defeat.
If not Sarem 2.0 or a clone of Sarem, an antagonist of Shepard with equal skill and charisma ( a clone made and controlled by Cerberus would make an interesting enemy).
The return of the "lost" teammate from ME as a repurposed Reaper agent, will Shepard redeem them or kill them?
More of Shepard'sr pre ME past playing an important part in the story.
NPC's being active and useful and talkative after their asignments have been done, new dialogue can be added after you finish a part of the mission.
Romance options for all persuasions.
Other characters than you could have romances as well, and personal lives too.
Customizing the Normandy with styles varying from warship to exploration/science vessel or anything in between.
Keep the FPS unaltered.
Add more locations.
Expand the existing locations.
Better control of the vehicles.
A three act story with a beginning, a build up and a resolution would work in ME3.
First act: Everything starts (relationships with the crew, romances, assignments and quests)
Second act: The build up of everything the with hints of the traitors existence being revealed and the plot thickening.
Third act: The resolution where evrything adds up, all plot points converge the traitor is exposed, the romance pays off and the crew of the Normandy unite against the enemy (or fall apart depending on the outcome of the loyalty missions and the in-ship side quests).
If Bioware is concerned about love scenes, then they could put the option of the blackscreen and the clothes-on scenes of ME2 or ME styled love scenes in the settings under censorship (high-low). That would grant content control to the player.
