You never really see the kill-shot of anything being beheaded in cut-scenes. If you kill Loghain, they show you Anora being showered in his blood. My guess? They didn't want to deal with figuring the animation on the head rolling.

For example, to geek out for a minute, season 7 of Buffy. Episode that involves the Bringer sneaking up behind Giles, swinging the axe with inches away from Giles's neck, and then the screen cuts to black. An episode or two later, Giles is still alive.
Nothing's official unless it's shown... or there's a blood splatter.

As for Loghain, he is a politician through and through.. What he did was right, but the King is not... er.. king-calible.. He may very well be thinking of the greater good.

Duncan's body gets picked up by this space chick that vaguely looks like michael jackson wearing spandex. She says she works for some 3-headed dog corporation or something and they fix him up. He's good as new, basically, but his face has lava scars. It's ok though, because the Warden ship can fix it if he's vain enough. Which, of course, he is. Primadonna.
